Home Based Primary Care provides interdisciplinary primary care services to eligible veterans in their home, in conjunction with family, community, and VA Medical Center support in an effort to maximize the patient's well-being and maintain the patient in the most independent setting possible. HBPC is expanding with an anticipated ADC of 300 patients in early 2023. Service area includes Longview, Monroe, Shreveport, and Texarkana.

Duties

Duties include: Assists in the planning, review, and reporting of data/statistical results of program/project studies. Performs a basic analysis of the data and generates a variety of reports. Will develop working knowledge of Home Based Primary Care (HBCP) Information Systems, VERA and DSS in order to maximize HBPC's efficiency and effectiveness. Develop and maintain spreadsheet data with patient information such as admission and discharge information, length of stay, acuity, IDC scheduling, referral source. Performs work related to the acquisition and/or development of program information and resource materials/personnel to support the policy development and/or technical activities of the organization. Performs work involving the collection, compilation, and/or tracking of data and statistical information in support of an organization's program operation. Completes special research projects in support of the policy development and/or technical/program activities of the organization. Will be specifically involved in projects, such as, Chart Audit Process, Performance Improvement Projects (for local HBPC and VISN HBPC collaborative projects), Infection Surveillance and Patient Satisfaction Surveys. Processes incoming and outgoing materials such as correspondence, ROI, reports, memorandum, CPRS/VISTA, P&O's and other forms of written communication. Provides general clerical mail correspondence services, such as preparing a variety of recurring reports, reviewing outgoing correspondence for proper attachments, or consolidating/coordinating submittals of information. Receives, refers, and contacts members of HBPC staff, patients, other VA staff, and persons outside the VAMC ranging from other government agencies to the general public. Acts as liaison to other managers and staff within the VAMC and provides accurate advice on procedures, reports, requirements, and other matters necessary to implement program policies, directives, and instructions. Provides general calendar maintenance clerical services such as maintaining calendars and recording appointments, arranging for meeting and small conference administrative details, or following up on action items with appropriate staff members. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/04/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement.. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-06 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Type correspondence such as memorandums, letters, reports, documents, etc. Advises internal and external customers appropriately and courteously. Perform customer service skills such as a receptionist, receiving telephone calls, routing calls to appropriate staff, greeting the customers, etc. Interprets oral and written communication and interpersonal relations, to appropriately and courteously relate to internal and external customers. Operate computers, printers, typewriters, calculators, and software programs such as MS Word, Excel and Office or compatible software. You will be rated on the following Competencies for this position: Customer Service (Clerical/Technical)Data SystemsDecision Making Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: The work is sedentary. Typically, the employee sits comfortably to do the work. However, there may be some walking; standing; bending; carrying of light items such as papers, books, small parts; or driving an automobile, etc. No special physical demands are required to perform the work.
