This position is aligned under the Fleet Medicine Directorate in the Public Health Department, located at the Captain James A. Lovell Federal Health Care Center (FHCC), North Chicago, Illinois; a 1c complexity facility. The Lovell FHCC is larger than just a single facility, but rather a system of facilities throughout Northeastern Illinois and Wisconsin which provide medical and dental services to U.S. Military Veterans, Recruits, and Active-Duty and eligible beneficiaries.
Duties
The Program Specialist (Health Promotion Coordinator) is responsible for the delivery of Veterans Health Administration (VHA) and Navy Health Promotion programming, wellness coaching, and skill development of the FHCCs DoD Health Promotion Program, including provision of care through coaching, health promotion, and education regarding monthly health topics to clientele (e.g., Active-Duty, Active-Duty beneficiaries, and FHCC staff members (i.e., VA civilian employees)). The position may support the planning and delivery of wellness programs for Veterans from time to time, though the primary target audience supported by this position is the Active-Duty population. The position is integral to supporting other Active-Duty and other wellness staff in navigating their health journey, specifically through the wellbeing and Navy Health Promotion Program clinical care elements as well as assisting in the collection of Navy Health Promotion qualitative data from activities, participation, and creation of reports analyzing effectiveness of health coaching and other programmatic activities. Duties include but are not limited to the following:
The position promotes health activities throughout the facility and Naval Station Great Lakes, Educates customers on the benefits of health coaching, specific wellness topics, and other health promotional programmatic activities, publicizing available resources, developing coaching clinics and groups, participating in outreach events.
Promote healing relationships, healing environments, and a patient-centered approach to care across all service lines; collaborate as needed with the other Whole Health Program Coordinators and other health and operational readiness leaders and liaisons.
Meets with external stakeholders to provide information about the Health Promotion Program and individual programs and access to clientele. Collaborates with internal and external resources for supporting the clients' health and wellness journey by developing resource lists for well-being support within the facility and in the local community.
Provides care to clients seeking self-directed, lasting changes aligned with their values, which are necessary for the physical and operational readiness.
Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Actively guides clientele in the exploration of core elements of personal health planning through individual and group-based programs.
Contacts eligible participants for enrollment in Health Promotion Groups, weight loss group classes, tobacco cessation services, or individualized health coaching, and explains the class, activity, service, or health coaching process. Provides health coaching for clientele to reduce or eliminate high-risk behaviors while increasing healthy ones.
Develops and conducts onsite educational series for enrolled clientele to explain health coaching, including its uses and benefits, and other Health Promotion activities, classes, educational opportunities available for staff, and service member participation.
Conducts group coaching/educating on health coaching and health related topics in the facility. Represents Health Promotion programs on decision-making committees within the Healthcare System. Participates in leadership roles on local, state and/or national professional organizations specific to health coaching and health promotion activities.
Promotes health activities throughout the facility, educating internal and external customers on the benefits of health coaching, specific wellness topics, and other health promotional programmatic activities, publicizing available resources, developing coaching clinics and groups, participating in outreach events.
Uses Electronic Health Records (EHR) (e.g., VISTA, CPRS, AHLTA, MHS GENESIS, CERNER products) or other patient-centered digital records information system, Microsoft office software, Intranet, Internet, and other applicable software to track and support patient progress as well as generate reports on activities.
Coordinates care with primary and specialty care providers to ensure identified goals are consistent with best practices for improvement of health, operational readiness, prevention of chronic illness, and long-term health problems. Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement,
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements. (If you have not held a GS grade at any time in the past 52 weeks, this question does not apply to you.) For this vacancy, applicants must have held grade GS-05 or higher for at least one year to be eligible for the GS-07 grade level of this position, or a grade of GS-07 or higher for at least one year to be eligible for the GS-09 grade level of this position. The grade must have been held in the Federal service but may have been in any occupation. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials.
If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

Specialized Experience GS-07: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to involves the planning, coordinating, execution of, and participation in process improvement activities for medical organizations (Hospitals/Clinics/Various Medically Related facilities) and Health Promotion Programs and individual programs to ensure that services are provided promptly, courteously, and with a compassionate understanding of clientele needs.

Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have 1 full year of graduate education or superior academic achievement. TRANSCIPTS ARE REQUIRED

Combination: Applicants may also combine education and experience to qualify at this level. You must have a combination of specialized experience and post high school education.
Specialized Experience GS-09: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Communication and interpersonal relations, including the ability to appropriately and courteously relate to internal and external customers; How to work and function in the medical center in a safe manner, including infection control and the correct operation of equipment; Knowledge of appropriate local, policies, directives, instructions, information flow, and prevailing automated practices sufficient to ensure applications are consistent with the current and planned infrastructure and data environments; Knowledge of health coaching structure, process, skills, and techniques; Knowledge of current research literature related to health and wellness, including behaviors that promote wellness and those that are detrimental; comprehensive reports, addressing elements such as patient reported outcome measures, work measurement, trend analysis, efficiency, effectiveness, and productivity; interpersonal skills to present recommendations that improve patient services.

Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another. Some physical effort such as standing, walking, bending or prolonged sitting may be required. The work places no special physical demands on the employee.
