This position is located in the patient Administration Department of the Facility Support Directorate located at the Captain James A. Lovell Federal Health Care Center (FHCC) in North Chicago, IL. The position serves as the PCMM Coordinator for the Primary Care and Mental Health teams within the FHCC service area including the Kenosha McHenry, and Evanston CBOC. The incumbent works in collaboration with the Clinic Coordinators, Providers, Nurses, Clerks and the rest of the clinic staff.

Duties

Major duties include but are not limited to: Program Management/Analysis (40%) - Works with Team Leaders and Clinic Managers to formulate and conduct training for all providers and clerks involved with PCMM and provides expertise and advice to all providers involved with PCMM. - Provides expertise and advice on an on-going basis to all clerks involved with PCMM - Trouble shoots and coordinates processes with all clerks involved with PCMM - Obtains, organizes files, retrieves/analyzes reports and analyzes numerical data for use in preparing status reports, briefings and presentations. Data Entry/Data Base Maintenance (40%) - Assigns/un-assign/re-assign patients to primary care and mental health teams daily. - Discharges patients from database when dis-enrolled from primary care or mental health daily. - Place ineligible patients on ineligibility list. - Identifies and corrects discrepancies in data base system. - Enters primary care and mental health team information into PCMM system. - Maintains accurate enrollment data. - Obtains, organizes analysis various files and retrieves reports and numerical data for use in preparing status reports, briefings and presentations. - Knowledge and access to CHCS & AHLTA. Report Generation (20%) - Generates the following reports bi-monthly: Inactivation, panel scrubbing, PCMM transmission error code, and MH performance measures. - Generates the following reports monthly: panel sizes, team assignments, no primary care assignment, unique patients assigned, staff inactivation, historical patient assignment detail, enrollment capacity, and current capacity. - Generate and work the following reports daily: Dual assignment, HL7 transmission, PCMM inconsistency, PCMM nightly tasks and provider assignment. - Creates templates for PCMM report generation. - Generates reports on a daily, monthly, or quarterly basis as requested Other duties as assigned: Monitors inactivation messages via Vista daily: reactivation of an automatically inactivated patient or extension or inactivation date. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/12/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-7 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Knowledge of qualitative and quantitative analytical techniques and the ability to apply that knowledge to evaluative methods and techniques to issues or studies concerning the efficiency and effectiveness of program operations; understanding of interface between clinical and administrative activities; knowledge of administrative regulations and operating procedures; skill in investigative techniques to gather factual evidence in support of compliance with regulations; knowledge of computer software programs equivalent to Microsoft Excel, Access, Outlook, PowerPoint, and Word processing, etc.; ability to prepare presentations, briefings, reports, correspondence, maintain databases and develop spreadsheets; ability to assist with budget analysis, personnel functions, costs and forecasting future needs; skill in dealing with diverse groups and individuals; and skill in oral and written communication and business writing and research methodology to compile and convey report data and provide recommendations. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have master's or equivalent graduate degree or 2 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related. OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond . You will be rated on the following Competencies for this position: Analysis and Problem SolvingCustomer ServiceData Management Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: Work is usually sedentary but requires walking, standing, and carrying of medium weight items, on occasion. Incumbent must have an even temperament and act in a professional manner during work performance.
