The incumbent will receive specialized training in Advance Care Planning group methodology and will establish ACP groups at the main facility and CBOC sites. The incumbent will supervise social workers at the GS-9, GS-11, and GS-12 level in the medical specialty areas, which includes but is not limited to neurology, traumatic brain injury, palliative care, hematology/oncology, spinal cord injury, and maternity care programs.

Duties

Duties to include but not limited to: - The incumbent is responsible for the administrative functions pertaining to coordination, program management, and oversight. The incumbent works closely with the Patient Aligned Care Team (PACT) Program Manager and Medical Specialty Care Assistant Chief of Social Work to ensure administrative and business functions are completed and the program is disseminated throughout the PACT system of care. - The incumbent functions as subject matter expert on VA and community resources related to Advance Care Planning and specialty medical clinic needs. The incumbent must be knowledgeable about the complex nature of medical, psychiatric, and psychosocial needs specific to veterans across the continuum of care. - The coordinator coordinates, schedules, and leads the implementation of the team, including outreach efforts, national reporting requirements, and dissemination of education. This includes updating the Advance Directive Medical Center memorandum policy. - The coordinator develops, writes, and revises local policy and procedures for the program to ensure adherence to VAC program standards and policies. This includes but is not limited to local medical center memorandums, program protocols, manuals, and brochures. - The coordinator establishes quality measures to be monitored regarding program performance for advance care planning providers as a part of providing quality assurance standards for program implementation. - Supervises social work staff assigned to the ACP and Medical Specialty areas. (i.e., hematology/oncology, neurology, traumatic brain injury, and/or palliative care). This includes monthly audits to monitor compliance with clinical case management requirements and adherence to program guidelines. - Must be a subject matter expert regarding VACO rules and regulations which govern ACP. - The incumbent is responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. - Other duties as assigned Work Schedule: Monday-Friday, 8:00am-4:30pm

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 01/02/2025. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-13 position you must have served 52 weeks at the GS-12. The grade may have been in any occupation, but must have been held in the Federal service. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part.. Preferred Qualifications: At least 2yrs or more of Supervisory Experience, Homelessness Experience, HUD-VASH Experience You may qualify based on your experience and/or education as described below: Grade Determinations: GS-13 Supervisory Social Worker: For Supervisory Social Worker, grades are determined by the scope of responsibility and the type, size, and complexity of the assignment. Experience, Education and Licensure: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below. Demonstrated Knowledge, Skills, and Abilities: a Ability to independently organize work, set priorities, and meet multiple deadlines. b) Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. c) Ability to ensure provision of clinical social work services by supervised social workers. d) Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion e) Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service. f) Ability to provide staff training and development.
