This position is located in Behavioral Health at the Phoenix Indian Medical Center (PIMC), in the Ambulatory Services Division. The incumbent is a Licensed Master Social Worker (LMSW) who serves as a Behavioral Health Case Manager. BH case management is focused on the provision of social work services that promote the efficient and effective use of a variety of health care resources to provide a seamless continuum of care.
Duties
Manages comprehensive care of outpatients and continuum of care for hospitalized patients at the facility through delivering full social work services designed to provide diagnostic, therapeutic, and specialized case management and discharge planning services to patients and families of the highest quality possible within existing resources.
Provides case management for patients by coordinating social and behavioral health services to patient's needs.
Screens potential patients identified through the triage and screening for Behavioral Health case management. Synthesizes information and determines the needed services.
Develops with the patient/family and interdisciplinary team members a care and service delivery plan based on needs identified and available provider and financial resources.
Reviews charts and treatment plans and contacts patient and service providers as necessary to determine if services are being delivered as planned, and if services are meeting needs.
Prepares patient and family for termination (discharge) from case management when services are no longer required.
Qualifications

You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
BASIC REQUIREMENT(S):
AMaster's Degree inSocial Work. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
GS-11: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-9 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Examples include: Performs intensive and wide range of behavioral health services including moderate technical difficulty including evaluation, consultation, counseling, referral, psychotherapy, crisis intervention and treatment of mental, emotional and behavioral disorders to patients and families.
OR
Successfully completed 3 years (54 credit hours) of progressively higher level graduate education leading to a Ph.D. degree or Ph.D. that is directly related to this field.
OR
A combination of education and experience, in one or more of the behavioral or social sciences equivalent to a major in the field, that when combined fully meet the minimum qualifications for this position. The total percentage equals at least 100 percent to qualify for this GS-11 grade level.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
