The Community Resource and Referral Center or CRRC is an off-site outreach program for homeless veterans in Phoenix, Arizona. The CRRC Program Supervisory Social Worker manages the administrative and clinical aspects of the CRRC. The incumbent has responsibility for coordinating interdisciplinary staffing, work assignments, budget, assessment, treatment, and referral services provided.

Duties

The CRRC Program Coordinator will be responsible for the administrative management and clinical leadership of the Intake/Outreach Team at the Phoenix CRRC facility with the Phoenix VA Healthcare System. The CRRC Program Coordinator directly supervises 15 total FTE; a multi-disciplinary staff which includes Social Workers, Addiction Therapist, Peer Support Specialists, and other Program/Administrative Support Assistants. The CRRC Program Coordinator is available for consultation and coverage for the hours of operation of the CRRC facility. The CRRC Program Coordinator is responsible planning, organizing and directing the administrative, operational, patient and non-nursing personnel activities for the CRRC programs. Duties include, but not limited to: Represent the Phoenix VA Healthcare System in collaborations with many Community Partners (i.e., federal/state/county governments, municipalities, providers of homeless services and other stakeholders). The CRRC Coordinator provides oversight of city and community providers located in the CRRC. CRRC Coordinator develops and implements standard operating procedures and policies to include but not limited to disruptive clients Responsible for the development of new community partnerships and participates in coordinating special events for Homeless Veterans such as Stand Downs and the Point in Time (PIT) Count. Coordinates quarterly meetings with VA departments and community providers / City to discuss service delivery and updates. Serve as a the VA/community champion for the Federal Initiative to End Veteran Homelessness, and responsible for overseeing all Federal, State, and Local initiatives related to that mission. Serves as a member of the Coordinated Entry subcommittee for the Maricopa County Regional Continuum of Care. CRRC Coordinator will be the point of contact for VA inspections / environment of care rounds, work orders, and equipment repairs and is responsible for ensuring all errors are corrected and reported to the responsible part. Participates in accreditation process for the CRRC to include Joint Commission and CARF. Responsible for program oversight, monitoring and administration, along with program participant data collection and reporting to the local, VISN, and national offices. Coordinate VA outreach in addition to all community outreach teams serving Veterans. Conduct ongoing intensive reviews and appraisals to ensure the accomplishment of objectives and adherence to standards set by governing bodies. Directly supervise a multi-disciplinary staff which includes Social Workers, Addiction Therapist, Peer Support Specialists, and other Program/Administrative Support Assistants. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Language: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a Master's Degree in Social Work from a school of Social Work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of Social Work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A Doctoral Degree in Social Work may not be substituted for the Master's Degree in Social Work. Verification of the degree can be reviewed in CSWE website to verify the Social Work degree meets the accreditation standards for a Masters of Social Work. Licensure. Must be licensed or certified by a state to independently practice Social Work at the Master's Degree level. Grade Determinations: Social Worker (Program Coordinator) GS-13 Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs. Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Knowledge of organizational structure and impact on program operations. References: VA Handbook 5005/120, Part II, Appendix G39 Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and standing, bending and carrying such items as books, papers, and files. In carrying out responsibilities it may be necessary for the incumbent to travel into the community where he/she conducts interviews with the Veterans, their families, representatives of the community. The incumbent must possess current driver's license and drive a government vehicle in carrying out professional duties when deemed necessary.
