The Social Worker Care Coordinator will be responsible for performing care coordination for patients enrolled in the Care Coordination and Integrated Case Management (CCICM) program. Care Coordination is a systemwide approach to the deliberate organization of all Veteran care activities between two or more participants or system to facilitate the appropriate delivery of health care services.

Major Duties for the position include (but are not limited to):
Responsible for integrating communication, collaboration, and coordination of care.
Serves as the primary point of contact for interdisciplinary team members to promote seamless Veteran care along the continuum, especially during transitions to other levels of care coordination and case management.
Creates a single care plan, in collaboration with all case managers involved in the Veterans care to address the Veteran's needs and goals across clinical service lines.
Exercise independent judgment and decision making in the implementation of treatment plans. Possess extensive skills in utilizing VA and community resources.
Conduct ongoing assessment of the Veteran's progress, as well as developing in-depth methods for determining the effectiveness of the program.
Serve as the Respite Care Social Work Coordinator working closely with Patient Flow to ameliorate or prevent social admissions due to complex social dilemmas, utilizing VA and non-VA resources to strengthen respite and social support systems for these Veterans.
Subject matter expert for the establishment of respite care and foster home providers in CBOC's located in Texas and Arkansas.
Facilitate the discharge of Veterans with unresolved complex legal problems.
Oversees the daily operation of Medical Foster Home Program and Respite Care Coordinator programs, including developing policies and procedures for program operation, preparing reports and statistics for the facility, the VISN, and national use.
Responsible for the budgetary oversight of these programs, developing and monitoring staff compliance with practice standards, guidelines on documentation, workload, data entry, ethical practice and service delivery.
Provides leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff.
Teach and mentor staff and students in the specialty area of practice to provide clinical social work services to veterans in Care Coordination and Integrated Case Management Program and other Institutional and non-institutional VA programs with high quality case management, clinical treatment, and advocacy.
Develops and implements information systems to track program activities including visits, referrals, patient demographics, service utilization and patient satisfaction.
Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities regarding VA CCICM, complex patients, and medical foster home services.
Ensures provision of an array of long-term care services to veterans who experience functional impairments from chronic conditions.
Work Schedule: 8:30am - 4:30pm, Monday - Friday

Pay: Competitive salary and regular salary increases.
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Candidates must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-12: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Applicant's must also meet the following Knowledge, Skills & Abilities (KSA):
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.

Physical aspects associated with work required of this assignment are typical for the occupation, see Duties section for essential job duties of the position. May require standing, lifting, carrying, sitting, stooping, bending, pulling, and pushing. May be required to wear personal protective equipment and undergo annual TB screening or testing as conditions of employment.
Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated, and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations. The incumbent who provides care in other off-site locations may be required to drive and/or ride in GSA-vehicles.
