The incumbent works closely with Patient Flow to ameliorate or prevent social admissions due to complex social dilemmas and will utilize VA and non-VA resources to strengthen respite and social support systems for these Veterans. This position conducts biopsychosocial assessments on Veterans who present in the emergency department due to a fall. The incumbent serves as the subject matter expert for the establishment of respite care /foster home providers at Overton Brooks Shreveport.

Duties

Duties include, but are not limited to: Maintains local level programmatic and operational responsibility for Respite service administration and coordination. Manages the daily operation of the Respite program, develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. Responsible for securing the service, utilizing the most appropriate settings and resources, whether VA or non-VA, inpatient or non-institutional care. Develops respite plan that may include a combination of inpatient admissions, as well as home or community-based alternatives, such as ADHC or in-home care. Responsible for providing reports, analysis, projections and recommendations on the program's budget within the assigned areas. Manages and controls the use of data related to Respite program regarding day-to-day functioning and program evaluation. In conjunction with the Social Work Chief, plans, organizes and directs administrative, operational, patient activities for the Respite program. When assigned, will teach and mentor staff and students in the specialty area of practice and provide clinical supervision for licensure. Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track program activities including visits, referrals, patient demographics, service utilization and patient satisfaction. Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Maintains responsibility for oversight of the respite service plan, and manages the program operation by providing administrative oversight and leadership. Coordinates services by interfacing with the referring treatment team, tracking and documenting referrals and prescribed episodes of respite services. Provides orientation to the veteran and the caregiver to the program. Monitors the veteran's respite plan of service. Ensures that the choice of service settings for the delivery of respite care reflects caregiver and veteran need. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Applicants must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as a social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, the candidate must demonstrate all of the following Knowledge, Skills, and Abilities: a. Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. b. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. c. Ability to supervise multidisciplinary staff assigned to the program. d. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). e. Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is GS-12. Physical Demands: Physical aspects associated with work required of this assignment are typical for the occupation, see Duties section for essential job duties of the position. May require standing, lifting, carrying, sitting, stooping, bending, pulling, and pushing. Ability to operate a motor vehicle. May be required to wear personal protective equipment and undergo annual TB screening or testing as conditions of employment. Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated, and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations.
