The Supervisory Pharmacist functions at the highest level of clinical practice, working independently under his/her scope of practice as defined by the medical center to directly care for patients requiring appropriate care in mental health and other specialties. The Pharmacist Supervisor is responsible for the professional and administrative management of an assigned area in pharmacy service. Serves as a consultant midlevel provider who functions to initiates and modifies medication therapy.

DUTIES INCLUDE BUT NOT LIMITED TO: Manages the activities of assigned Clinical Pharmacy Staff and has the oversight of ongoing inpatient and outpatient clinical pharmacy services. Works under the supervision of the Associate Chief, Operations and, as adjunct, Associate Chief, Clinical and Education Services. Works with other pharmacy staff to assure that pharmacy clinical services are well coordinated and incorporated into the overall approach to patient care. Assures that the pharmaceutical care needs of patients are met across the continuum of care within the scope of their clinical specialty area. This supervisor will oversee all mental health CPPs and others to be determined. They will also maintain an active clinical practice in Mental health as a clinical pharmacist practitioner. The incumbent has full administrative authority over subordinates in his/her area of specialty, and ensures that services provided meet or exceed the professional and legal standard. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Ensures implementation of performance standards, assures that clinical practice is compliant with all regulatory and accrediting body requirements. Develops policies and procedures relative to the assigned area of practice. Designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Interviews prospective candidates for appointments, recommends appointments, advancements, or promotions or disciplinary actions. Incumbent will have direct or indirect oversight of the Pharmacy Residency Program and other educational programs if applicable to his/her area of practice. S/he may serve as a Residency Program Director (RPD) if a residency program is established at their site and if selected for the position of RPD. The incumbent will operate under a broad delegated authority from the medical center. Supports the Chief of Pharmacy as needed during various inspections and surveys such as Joint Commission. Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor. Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required. Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the work day. When required, communicates any problems to the medical staff and/or other health care professionals, and offers alternatives or solutions to the problems. Demonstrates recognized expertise in the assigned area of practice for the purpose of management of several disease states specific to his/her area of clinical specialty. These specialties include-but not limited to - diabetes, hypertension, hyperlipidemia, anticoagulation, antimicrobial stewardship, spinal cord injury, traumatic brain injury, surgical/transplant care, and acute/critical care pharmacotherapy issues. Manages all patients receiving anticoagulation therapy in all medical settings if applicable to his/her clinical practice specialty unless otherwise requested by the attending physician. May also participate as a provider in the outpatient Anticoagulation Management Clinic. Performs at least one hospital committee assignment per year, or attends professional meeting and presents a report to the facility's professional staff and documents committee contribution or impact of meeting. Demonstrates knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to his/her practice area as applicable. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization. In cooperation with clinical section staff and other health professionals, collects and analyzes data related to ongoing clinical programs and presents it either at national meetings or publishes it in professionally representative publications. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT). Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). A sustained record of contribution and commitment to pharmacy, as defined by ASHP Meets criteria to work under a scope of practice Completion of a Pharmacy Practice or Specialty Residency with substantial component (>50%) of direct patient care activities involving comprehensive medication management OR, Completion of a minimum of 3 years of pharmacy practice experience with a substantial component (>50%) of direct patient care activities involving comprehensive medication management Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
