This clinical pharmacy practitioner plays a defined role in budgetary execution and serves as a consultant and midlevel provider who functions to initiate, modify or discontinue medication therapy, with an emphasis on psychiatric medications. Extensive documentation and clinical progress note writing are required of this position. S/he may be involved in research activities and precepting of pharmacy trainees.

Duties

Duties to include but not limited to: Coordinates all clinical pharmacy functions for his/her assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in team meetings, conferences, and other scheduled events as required. Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor. Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required. Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the work day. When required, communicates any problems to the medical staff and/or other health care professionals, and offers alternatives or solutions to the problems. Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Monitors patients prospectively for any deviation from the accepted treatment plan, for the efficacy of the plan, and for any adverse reaction or drug interaction occurring during the execution of the plan. Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics and pharmacotherapeutics as determined by peer review. Reviews, analyzes, and applies literature to specific patient situations. Documents interventions and recommendations via CPRS progress notes and consults, demonstrating the ability to communicate orally and in writing to persuade and influence clinical and management decisions. Written documentation of all patient interactions is required. Assists the area of practice in meeting all of The Joint Commission medication management standards by having an expert understanding of regulatory and quality standards for the area of practice. Supervises pharmacy trainees and residents assigned to participate in the program. Participates in other clinical pharmacy activities as assigned by supervisor. Participates in P&T Committee functions and assignments as required, e.g., ADR monitoring, medication error reporting, medication utilization evaluation activities, drug monographs/reviews, etc. Informs clinical coordinator of changes in nature of assigned duties which could affect clinical activities. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT). Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Meets criteria to work under a scope of practice 1. Completion of a Pharmacy Practice or Specialty Residency with substantial component (>50%) of direct patient care activities involving comprehensive medication management OR, 2. Completion of a minimum of 3 years of pharmacy practice experience with a substantial component (>50%) of direct patient care activities involving comprehensive medication management. OR, 3. Receipt of recognized board certification through the Board of Pharmacy Specialties (BPS) References: VA HANDBOOK 5005/55PART II APPENDIX G15, LICENSED PHARMACIST QUALIFICATION STANDARD The full performance level of this vacancy is GS-13. Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
