Pharmacogenomics (PGx) is relevant to a broad range of therapeutic areas (e.g., Cardiology, Oncology, Pain, Psychiatry, Neurology, Infectious Disease, Transplant). PGx has demonstrated a significant impact on avoidance/prevention of adverse drug events and has the ability to prevent ineffective/harmful initiation of drug therapy, and/or continuation of drug therapy that is not optimal in patients who may be identified with genotypes that have associated actionable drug-gene pairs.
Duties
DUTIES INCLUDE BUT NOT LIMITED TO:
Establish prevention and management of adverse drug events related to therapy based on pharmacogenomic results and drug-gene pairs, paying special attention to medications that are high-risk for adverse events
Support implementation and future opportunities for sustainment of clinical PGx testing as an integral and routine part of CMM
Utilize population health management tools to enhance CMM to improve safety and efficacy of medication regimens
Develop processes for implementation of a clinical pharmacogenomic service/stewardship program, as part of a multidisciplinary team
Serve as a SME and/or program lead on a clinical pharmacogenomics service/stewardship program, once established
Collaborate with the National Pharmacogenomics Program Office and other pharmacogenomic programs to support initiatives, goals and local/VISN champions
Develop and revise policies, clinical pathways, order sets and menus that promote the optimal use of pharmacogenomics testing (i.e., pre-emptive/reactive) based on published guidelines and primary literature
Contribute to the development and sustainment of pharmacogenomic-specific clinical decision support (CDS) tools (i.e., passive and active) in electronic health record systems that guide prescribers on the appropriate use and dosing of medicines based on a patient's pharmacogenomic profile 1
Educate patients on PGx principles to establish realistic expectations so that patients can make a value added decision to participate in PGx testing
Assist the facility laboratory and informatics teams to identify and incorporate infrastructure to support pharmacogenomics laboratory services and to track and report on improved patient outcomes of the facility PGx, such as avoidance of adverse events and inappropriate medication selection
Document PGx interventions utilizing standardized templates with health factors developed by the CPPO PGx workgroup for data tracking and trending of value-added PGx interventions
Develop processes to document economic benefits resulting from clinical pharmacogenomics
Develop education for patients and VA clinical staff about PGx principles and the appropriate use of PGx testing and application to guide medication therapy
Serve as a preceptor for advanced experiential rotations for pharmacy practice residents and students, looking for pharmacogenomic training opportunities

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT).
Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice.
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
A sustained record of contribution and commitment to pharmacy, as defined by ASHP.

The full performance level of this vacancy is GS-13.
Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
