The Neurology Clinical Pharmacist Specialist functions independently with prescribing privileges under a scope of practice as defined by the medical center to directly care for patients served by the Neurology service and other areas in the VAMC. The Clinical Pharmacist Specialist is responsible for planning, developing, and maintaining highly innovative and progressive clinical pharmacy programs by implementing rational cost effective neurology drug therapy to improve patient outcomes.

Duties

Duties to include but not limited to: Coordinates all clinical pharmacy functions for assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in multidisciplinary rounds, team meetings, conferences, and other meetings as required. Demonstrates recognized expertise in the assigned area of practice for the purpose of management of several disease states, including-but not limited to-headache, epilepsy, movement disorders, amyotrophic lateral sclerosis (ALS), multiple sclerosis (MS), stroke. Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required. Evaluates non-formulary and restricted medication requests from assigned providers. Supervises pharmacy trainees and residents assigned to participate in the program. Participates in other clinical pharmacy activities as assigned by supervisor. Gathers data for administrative reports when assigned. Upon request, independently prepares reports or participates in pharmacy projects. Assists in preparing for special accreditations and reviews (e.g. The Joint Commission, Inspector General, etc,) and meets with reviewing/inspecting officials and pharmacy management. Demonstrates knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to practice area as applicable. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization. Develops and promotes a "can do" resourcefulness helping patients, relatives, staff members and others within and between services by providing information in a professional, dependable, reliable and timely manner to facilitate accomplishing deadlines. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT). Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Meets criteria to work under a scope of practice 1. Completion of a Pharmacy Practice or Specialty Residency with substantial component (>50%) of direct patient care activities involving comprehensive medication management OR, 2. Completion of a minimum of 3 years of pharmacy practice experience with a substantial component (>50%) of direct patient care activities involving comprehensive medication management. References: VA HANDBOOK 5005/55PART II APPENDIX G15, LICENSED PHARMACIST QUALIFICATION STANDARD The full performance level of this vacancy is GS-13. Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
