The Clinical Pharmacist/Program Manager manages all pharmacy related activities of VA authorized community care programs and ERx (electronic prescriptions from the community) with regard to medication order review and processing. The Pharmacist plans and coordinates pharmacy services for these VA community care programs patients and providers within the guidelines of each individual program guidelines, Joint Commission standards, and medical center policies/regulations.
Duties
Duties to include but not limited to:
The incumbent must be able to demonstrate knowledge of current pharmacy laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. The incumbent is responsible for addressing a variety of questions posed by physicians, nurses, coworkers and patients related to drugs, pharmacy services and departmental policies and procedures. He/she must be able to interpret and apply VA, federal, and state regulations, Joint Commission, DEA, FDA, hospital and Pharmacy Service polices under all circumstances.
The incumbent must have the ability to exercise sound judgment in a variety of situations and refer matters of policy making to a supervisor.
The incumbent is required to maintain current knowledge and demonstrate professional pharmacy practice. The incumbent is required to complete all education and training requirements by the VA, VISN, and medical center by the assigned due date or as required by the Chief of Pharmacy.
Provides pharmacist oversight for VA Community Care Programs by reviewing and entering outside prescriptions written by VA Community Care Programs providers for patients enrolled at the medical center.
Serves as consultant and information resource to the VA Community Care Programs physicians and applicable medical center staff.
Ensures patient prescriptions written by outside providers conform to both national and local VA formulary and adjudicates non-formulary requests if necessary.
The incumbent provides direct clinical service for medical center patients, caregivers, and providers when therapeutic interchanges are mandated / deemed prudent. This may be achieved either verbally or via written correspondence.
Documents and reports drug cost savings relative to therapeutic interchange to the Assistant Chief, Education and Clinical Services.
Regularly documents recommendations and interventions.
Provides refill extensions and partial medication supplies when appropriate.
Interprets health and medication histories, performs medication reconciliation, and provides drug information.
Develops and promotes a "can do" resourcefulness helping patients, relatives, staff members and others within and between services by providing information in a professional, dependable, reliable and timely manner to facilitate accomplishing deadlines.
Provides timely verbal and/or written information that is thorough and correct, using clear and precise communication methods to ensure that patients and other health care providers understand.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT).
Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice.
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

The full performance level of this vacancy is GS-13.
Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
