The Lead Medical Foster Home (MFH) Coordinator is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence-based services and interventions, program development, program management, community outreach, resource development, continuous quality improvement activities, evaluation/consultation, and MFH caregiver, Veteran, and staff education.

Duties

Duties include but are not limited to: Manages and directs multi-program staff and ensures that all Home Based Primary Care (HBPC) and MFH policies are followed. Develops SOPS for both HBPC and MFH Programs. Manages workload of MFH/HBPC Programs and travels to the 6 clinic locations as needed. Coordinates HBPC SW and MFH services with VA PACT teams, assessing new referrals and admitting new Veterans to the Program. Provides oversight of MFH, SW, and RT services on 9 HBPC PACT teams. Administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families and Veterans within the MFH and HBPC Programs. Develops and implements information systems, including clinical guidelines and protocols, to track and establish service activities including visits, referrals, MFH and HBPC caregiver demographics, equipment utilization and caregiver and Veteran satisfaction. Performs work related to the review, monitoring, analysis, planning, and assessment of MFH and HBPC programs. Works closely with senior management and others in implementing and evaluating policies, plans, and internal operational systems such as correspondence management, information tracking, reports and statistics for the facility, VISN and national use. Performs other related duties as assigned. Advises, consults, and collaborates with interdisciplinary teams throughout the medical center on MFH and HBPC related issues. Creates educational tools, develops programs and implements training focused on specific HBPC/MFH Veteran and caregiver needs/issues. Provides education and in-service training to VAMC employees, Veterans, families and caregivers. Provides ongoing consultation and support to colleagues regarding HBPC and MFH related topics. Develops and facilitates MFH caregiver support groups (face-to-face or VANTS telephone line) and educational programs. Provides referral to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. Stays abreast of current federal and state legislation directed at assisting MFH caregivers to continue to provide caregiving in an environment that is safe for both themselves and the Veteran. Develops a library of Veteran and MFH/HBPC caregiver support information material for distribution to Veterans and caregivers. Serves as a liaison between the VA and community agencies. Participates in caregiver support activities in the local community and state to promote and recognize the role of the caregiver and to market the MFH and HBPC Programs. Participates on state and local caregiver support committees or other committees pertaining to Veteran care. Establishes and maintains access to electronic social networks such as Facebook, Twitter, secure email and YouTube in accordance with VA policy. Develops and fosters relationships with community partners to assist with system navigation, maintain collaboration and coordinate services. Provides outreach presentations to community social service agencies, military Guard and Reserve units, Veteran Service organizations and other groups regarding HBPC and MFH services. Provides consultation, guidance and expertise to community organizations regarding Veteran and caregiver specific issues and needs. In collaboration with the with other clinical staff and program managers will establish and implement a MFH caregiver support training sensitive to the issues of the caregivers who are providing care to our Veterans admitted to MFHs. Develops a data monitoring system to measure the efficacy of the program. Collects, analyzes and reports program data to local management,Social Work Service, and VACO MFH Director. Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and processes. Performs a wide range of services and provides technical assistance to state, local, and private agencies, and universities. Uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.). Basic Requirements: Citizenship - Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g Education- Have a master's degree in social work from a school of social work fully by the Council on Social Work Education (CSWE). Graduates from school of social that are in candidacy status do not meet this requirement until the School of Social Work fully accredited. A doctoral degree in social work may not be substituted for the master's in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensure/Certification -Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-13 Social Worker (Program Coordinator) candidates must have the following: Experience/Education: One (1) year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. (c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. (e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. (f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. (g) Knowledge of organizational structure and impact on program operations. References: VA Handbook 5005/120 Part II Appendix G39 Social Worker Qualification Standard. The full performance level of this vacancy is GS-13. Physical Requirements: The physical demands of the work are generally minor. The work is primarily sedentary in climate controlled clinical and administrative settings. The incumbent will be expected to successfully negotiate various functional requirements: lifting (15-44 pounds), carrying (15-44 pounds), walking (1-3 hours), reaching above their shoulder(s), the use of hands or fingers, sitting (4-6 hours), standing (1-2 hours), repeated bending, using stairs or elevators, functional vision, clear speech, hearing, and emotional stability. There may be occasional requirements to relocate boxed materials from one area to another, equipment and room set-up, and physical inspections of the facilities.
