The Clinical Pharmacist functions at the highest level of clinical practice, working independently under his/her scope of practice as defined by the medical center to directly care for patients served in all critical care wards (MICU,CCU,SICU) and any acute care wards assigned. This Clinical Pharmacist plays a defined role in budgetary execution; Serves as a consultant for intensive medication therapy management and midlevel provider who initiates, modifies or discontinues medication therapy.

Duties

DUTIES INCLUDE BUT NOT LIMITED TO: Coordinates all clinical pharmacy functions for the critical care wards (MICU/CCU/SICU) and acute care wards as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in multidisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment. Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the work day. Assists the area of practice in meeting all of The Joint Commission medication management standards by having an expert understanding of regulatory and quality standards for the area of practice. Demonstrates knowledge of the standards related to Hospital services and Acute/Critical Care units. This includes, but is not limited to, the standards established by DEA, FDA, VA, the state, and The Joint Commission. Participates in P&T Committee functions and assignments as required, e.g., ADR monitoring, medication error reporting, medication utilization evaluation activities, drug monographs/reviews, etc. Provides formal education and clinical education in the area of expertise to pharmacists in training. Gathers data for administrative reports when assigned. Upon request, independently prepares special reports or participates in special pharmacy projects. Assists in preparing for special accreditations and reviews (e.g. The Joint Commission, Inspector General, etc,) and meets with reviewing/inspecting officials and pharmacy management. Participates in audits or quality management monitoring as demonstrated by provision of comprehensive quality assurance monitoring reports and completes at least one QA monitor yearly. S/He assumes complete responsibility for Critical/Acute Care pharmacy quality assurance audits, and gathers data for or performs Critical/Acute Care drug use evaluations to assess and improve the quality of patient care for the P&T Committee. Demonstrates knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to Critical/Acute Care as applicable. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required ensuring appropriate and safe drug utilization. Develops and promotes a "can do" resourcefulness helping patients, relatives, staff members and others within and between services by providing information in a professional, dependable, reliable and timely manner to facilitate accomplishing deadlines. Demonstrates a concerned, compassionate and helpful attitude to enhance a positive image of the Medical Center and the VA system when interacting with patients, relatives, staff members and other individuals dealing with the Medical Center. Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations or changes (within scope of clinical privileges) as necessary. Frequency and timeliness of reviews will be determined by The Joint Commission, and /or other regulatory requirements. Informs clinical coordinator of changes in nature of assigned duties which could affect clinical activities. Serves as preceptor of residents' research projects as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT). Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Meets criteria to work under a scope of practice 1. Completion of a Pharmacy Practice or Specialty Residency with substantial component (>50%) of direct patient care activities involving comprehensive medication management OR, 2. Completion of a minimum of 3 years of pharmacy practice experience with a substantial component (>50%) of direct patient care activities involving comprehensive medication management. OR, 3. Receipt of recognized board certification through the Board of Pharmacy Specialties (BPS) References: VA HANDBOOK 5005/55PART II APPENDIX G15, LICENSED PHARMACIST QUALIFICATION STANDARD The full performance level of this vacancy is GS-13. Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items. The work also requires the use of fingers, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted).
