The Community Health Program coordinates skilled care to Veterans purchased from outside community vendors. The program works collaboratively with local VA Medical Center providers, providers at other VA Medical Centers and with local community vendors, including home care and hospice agencies, local hospitals, long term care facilities, etc.
Duties
The incumbent will be responsible for telephone and written correspondence with providers, outside vendors, and local community hospitals and agencies.
The incumbent will maintain program supplies and equipment, including independently identifying needs and making acquisitions. Places appropriate orders and tracks requests through delivery.
The incumbent will be responsible for receiving, tracking and appropriately forwarding or processing incoming outside medical records, treatment plans, outside provider orders.
The incumbent will be responsible to establish a monitoring system for tracking, admission, discharges, authorizations and extensions, non-VA services purchased through the Community Health Program.
The incumbent will be responsible for collecting, compiling and tracking statistical information on to support the program operations, including productivity/workload reports, patient/ purchased care tracking reports

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/11/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Knowledge of VA policy, regulation, and mission, vision, philosophy to assure all actions conform with regulatory requirements. Knowledge of data collection methodology, tools, and techniques, as well as basic data management and analysis, including basic statistical methods and formulas to calculate measures of central tendency, frequency distributions and to present a variety of statistical data in tabular and report format. Ability to communicate verbally and in writing to different audiences, different background levels of education and understanding. Knowledge of a broad range of patient care and administrative services at the Orlando VA Medical Center necessary to plan, implement, perform various job requirements. Knowledge and expertise in Outlook, CPRS, Microsoft Office, including Excel, Word, Access, and other software programs to input, retrieve, and process data as required for tracking, trending, and reporting program data. Ability to analyze and interpret data, recognize errors and deviations from norm, including database, spreadsheet, and survey data. Knowledge of medical terminology, medical record components. Knowledge of scheduling package for scheduling VA clinic follow up appointments.

Physical Requirements: The work is primarily sedentary; typically, the incumbent is comfortably seated to perform required work. The position requires some physical effort, including standing, walking, bending, filing, liftin?? or carrying light weight items such as files, mail, and supplies in and through all parts of the facility. The incumbent is required to answer numerous telephone and personal inquiries and must have adequate eyesight, manual dexterity to allow for reading, operating computer, fax, printer and other similar office equipment.
