This position is located at the Elko Service Unit, in Elko, NV. The incumbent is a member of an ambulatory medical home care team with both medical provider staff and other health care staff. Incumbent ensures patients at the Health Center receive quality patient centered care that is consistent with the patient-centered medical home principles, and that care is timely, appropriate, cost effective, sensitive and coordinated to affect desired clinical outcomes.

Duties

Implements and initiates standardized disease management and health prevention guidelines across the patient care continuum. Responsible for setting up and monitoring patients enrolled in disease specific home monitoring and communicating clinical information and changes to clinical plan between patient and provider/care team. Develops a plan of care with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s). Manages and works with clinical and non-clinical health care staff to ensure improvements in access to care. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. BASIC REQUIREMENT(S):Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree From Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above, many positions may require experience in a specialty area of nursing. MINIMUM QUALIFICATIONS:GS-10: At least one (1) full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples include: Management of nursing care in inpatient/outpatient primary care environments and/or case management, emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor and evaluate health care options and services. Experience in coordination of patient care and resources for a caseload of patients across the continuum of care and through care transitions; Able to provide patient and family education pertinent to a wide range of conditions and health factors. GS-11: At least one (1) full year of professional nursing experience (equivalent to the GS-10 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples include: Care Management involving patient assessment, initiating, and maintaining individual multidisciplinary plan of care involving patients and their families to give optimal care. Ability to make decisions and implement them in complex health care situations. Extensive experience in nursing practice, service, and education; make independent judgements related to assignments, coverage and supervision; provide training and mentor nursing staff and provide input on evaluations. OR Education: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. You must meet all qualification requirements within 30 days of the closing date of the announcement.
