This position works with the Diabetes Management Department located at the Elko Service Unit in Elko, NV. This position teaches, develops, monitors, and evaluates the diabetes education program. This position reports to the Nurse Executive at the service unit.

Diabetes education:
Provide diabetes self-management education:
Facilitate diabetes-related learning and lifestyle change by persons with diabetes, family members, persons at risk for diabetes, and members of the general community.
Assess individual learning needs; develop and implement a plan for diabetes learning and behavior change specific to individual needs; document teaching and encounters; evaluate care plan outcomes; and modify/revise approaches as needed.
Teach patients and family members on a 1: 1 and small group basis; predominant settings include in-hospital and outpatient facilities, however in-home and community interventions may be indicated.
Participate in annual review and revision of the diabetes self-management education curriculum.
Diabetes Support Services:
Work with Diabetes Management staff, members of the Diabetes Clinical Care Team and the goal of diabetes care and improving
patient outcomes. Contribute to the ongoing improvement of GPRA diabetes outcomes.
Identify and analyze problems on an ongoing basis; plan and implement approaches to enhance care; and evaluate services.
Implement performance improvement activities; review results and perform indicated/ongoing follow-up
Work with individuals from relevant departments to address the diabetes services provided by that department.
Facilitate the coordination of patient care among multidisciplinary care providers.
Participate in diabetes program plan activities.
Adjust diabetes medication dosages between medical provider visits following clinical guidelines.
Qualifications

BASIC REQUIREMENT(S): Individual Occupational Requirements

Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

MINIMUM QUALIFICATIONS: GS-11:
Your resume must demonstrate at least one (1) full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples Include: Participation in teaching, developing, monitoring , and evaluating the diabetes education program; general health coaching for patients without diabetes as therapeutic lifestyle coaching, Assist with the provision and coordination of components of service unit diabetes care and education. Utilizes existing and self-developed teaching tools and course material and class population and consults with other members of the educational department team for their input into specialized training programs. Assesses learning and training needs of newly licensed nursing staff and contract/agency staff utilizing a variety of tools from which recommendations for in-service education, specialized training, individualized training, and new and improve training programs can be made. Collects and organizes data obtained from assessment methods. Teaches and/or directs and coordinates others to teach utilizing new or adapted approaches, Utilizes appropriate teaching methodology and leveling techniques to ensure course effectiveness and proper learner responses.
OR
Successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
