The Social Worker (High Reliability Organization (HRO) Coordinator, Program Director has full responsibility for strategic planning as well as coordination (development, implementation, monitoring and evaluation) of the Medical Center facility activities that relate to HRO journey for a level 1 healthcare system.

Duties

The Social Worker (HRO) Coordinator, Program Director uses Quality Improvement, Patient Safety, and Systems Redesign (Lean) skills to foster the development of a High Reliability Organization (HRO) at the unit/service/medical center level. The HRO Program Director provides the overall direction and consultation for the HRO principles across the facility. Duties include planning and organizing developments within the facility as well as evaluating, advising and coordinating administrative and clinical activities. This includes management improvement and education to deliver strong HRO principles across the culture of the entire organization. Also, mentorship and partnership across all service lines and all levels of staff in the organization are inherent. Ensures sustainability and spread of efforts directed at improving quality, safety, effectiveness through utilization of HRO principles of preoccupation with failure, reluctance to simplify, sensitivity to operations, commitment to resilience and deference to expertise. This position is embedded in the Quality & Patient Safety service, and reports to the Chief of Quality & Patient Safety, with a strong tie to the Directors Office and their pursuit for HRO. Performs other duties as assigned related to quality/performance improvement. Serves as Chair of the Facility HRO Work Group. Communicates the VISN HRO Strategic Plan to the Facility HRO Work Group. Coordinates and facilitates activities consistent with the VISN HRO Strategic Plan within the facility. Provides regular guidance and oversight to the Facility HRO Workgroup. Leads formal meetings with all members of the Facility HRO workgroup Communicate regularly with the VISN and ensures attendance at the Facility HRO Workgroup monthly meetings conducted by the VISN. Participates in Facility site visits conducted by the VISN and/or the National Center for Patient Safety (NCPS) to review and promote HRO activities. Ensures Facility participation in the VHA Patient Safety Program as outlined by NCPS and VHA Handbook 1050.01. At a minimum ensure: A robust RCA program, A defined method to monitor and improve safety culture and A robust CRM program Breaks down silos and other perceived barriers between services to ensure development of strong patient care and administrative processes. Works, collaboratively with the Quality Manager, Patient Safety Manager(s), and Systems Redesign to identify vulnerabilities and develop processes and programs that advance patient care across the enterprise. Places the Veteran as the focus of all process innovation and change. Shares lessons learned across disciplines, settings, and in a broader sense, outside of the facility. Assists the Medical Center in implementing priority sections of the High Reliability Organization (HRO) Toolkit. Ensure the ongoing review of pertinent data related to the Patient Safety Program and HRO activities to include but not limited to: o JPSR and SPOT data base reports o NCPS Quarterly Reports o NCPS Patient Safety Culture Survey (PSCS) o Patient Safety Assessment Tool (PSA T) o Patient Safety Program Site Visit Reports o All Employ Survey (AES) o Employee Engagement Survey o Inpatient Evaluation Center (IPEC) Reports o Joint Commission and other Regulatory Survey Reports Performs other related duties as assigned in functional statement. Basic Requirements: Citizenship: Be a citizen of the United States Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Physical Requirements. Candidates must be proficient in spoken and written English to be May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-13 (1) Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Skill in coordinating and implementing one or more specialty program(s), across the facility. (b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. (c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. (e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. (f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. (g) Knowledge of organizational structure and impact on program operations. Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent. At this grade level, social worker program coordinators oversee the administrative and clinical aspects of a major specialty treatment program. References: VA Handbook 5005/120, Part II Appendix G39 The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is the GS-13 grade level. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
