The Salisbury VAMC is seeking one Home Based Primary Care Director.
Duties
The Home Based Primary Care (HBPC) Program Director demonstrates advanced practice skills and judgment. The HBPC Program Director is licensed by a state to independently practice social work at the master's level and coordinates/directs the administrative and clinical aspects of a major specialty treatment program (Home Based Primary Care). The incumbent has responsibility for coordinating and directing the HBPC Program, including providing clinical supervision for interdisciplinary staffing, work assignments, budget, treatment services provided, and implementing policies and procedures that promote quality of care. The incumbent may supervise administrative employees assigned to the primary program and GS-9/11 clinical social work staff. The incumbent is a member of the VISN GEC Sub-council and VISN 6 HBPC Directors and provides presentations to these groups accordingly. The incumbent participates in VACO and VISN workgroups and planning committees, as well as mentoring other programs as requested.
Duties include but are not limited to:
Be responsible for the day-to-day operation of the HBPC program. This includes planning, directing, budgeting, monitoring, and evaluating the program. Determine HBPC program goals, working independently and with Geriatrics and Extended Care (G&EC) management in the development of plans for the attainment of these goals.
Identify program resource requirements, ensure appropriate acquisition and allocation of essential resources and account for the effective utilization of available resources in support of program activities. Identify and implement changes in the organizational structure of the HBPC program essential to ensuring confluence with changes in the treatment needs of the patient population.
Consider, from an informed perspective, a broad spectrum of factors when making programmatic decisions or recommendations to G&EC management including labor-management relations, public policy positions, effects on other VA and non-VA organizations/activities, economic impact, and other similar such considerations.
Ensure effective integration of program efforts/activities with other medical center operations and with community based agencies and activities.
Establish, comprehend, interpret, and effectively communicate program emphasis, operating policy and overall treatment philosophy to other operational entities.
Effectively manage general personnel policies and established practices regarding program operations as they relate to key employees and management actions with possible serious repercussions.
Ensure compliance with local and system-wide program policy manuals and directives.
Serve as resource person in the medical center for questions regarding Home Based Primary Care.
Demonstrate leadership in addressing ethical issues that impact patients and staff in or beyond the organization and in the local health care community.
Develop educational program for staff and students of various disciplines including fellows and residents. Forecast new knowledge needs for changing environment. Plans, implements and evaluates strategies to meet those needs.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs.
Skill in coordinating and implementing one or more specialty program(s), across the facility.
Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
Knowledge of organizational structure and impact on program operations.
