This position is located at the Samuel S Stratton VA Medical Center (Albany VAMC). The incumbent is responsible for all decisions related to the administration of community care and further assures complete clinical integration of Veteran care received in the community with Veteran care provided by staff at Albany VAMC. He/she advises the Chief of Staff on all matters related to interpretation of legislative and VHA wide policies for Community Care.

Duties

Major duties include but not limited to: Responsible for all aspects of Community care occurring at VAPORHCS, including: Advising Medical Center Director, senior medical center executives, physicians, and division staff on matters relating to all legislative and policies regarding Community Care. Serving as the subject matter expert for Community Care across the medical center. Provides leadership both internal to the service and across the medical center that ensures a well-balanced blend of solid patient care and sound financial choices/decisions relative to the management of purchased care. The incumbent is responsive to national and VISN mandates and initiatives relative to purchased care. Establishing local policy within the context of national directives and legislation regarding the implementation of purchased care activities. Provides direction regarding application of eligibility regulations, access to purchased care services within the context of a finite budget environment, consistent clinical care coordination/case management of purchased care episodes and ensures that a consistent quality of care is delivered to all patients participating in the Community Care program. Researching and/or analyzing program issues and/or requirements. Based on changing operational and program requirements, identifies relevant issues, and provides expert analysis and advice on complex program issues. Collects and assesses relevant information from many varied sources, some of which may be difficult to access or that may be controversial in nature, to make strategic recommendations to medical center management. Performing regular demographic and market analysis of purchased care supply and demand in Albany's catchment area. Ensures proactively Albany's ability to respond to changing demands for purchased care services. Assessing and developing strategic initiatives that respond to Albany's future goals and enhances the medical center's financial effectiveness. Proactively identifies and provides cost/benefit and make/buy analyses that represent improved patient care, revenue opportunities, and/or increased fiscal efficiency. Collaborating with service leadership and medical staff across the medical center to facilitate the development of Community care programs and processes responsive to patient care needs. Provides opportunity for regular feedback between clinical (MD) staff and purchased care staff regarding patient care hand-offs, as well as volume/cost data. - Analyzing management techniques, processes, and styles for improving organizational effectiveness. Conducts management improvement studies that improve the quality of non-VA care veteran patients receive and/or maximizes the funds available for non-VA care. Serves as a liaison for the medical center with external medical providers and clinical groups. Establishes resources for sending Veteran care into the community that is of high quality, reliable, and timely. Builds and maintains collaborative relationships with community suppliers of multiple entities, to assure every opportunity for Veteran care is timely and of high quality. Recognizing the competitive environment of accessing healthcare in the community, establishes partnerships that will withstand the ups and downs of the price/supply/demand equations. Monitors quality of Community Care, implementing clinical monitors and assuring complete analyses of any patient care errors. Special projects or reviews as needed in the Office of the Chief of Staff or across the organization. Performs other duties as assigned. Performs other related duties as assigned. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 01/13/2025. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-12 position you must have served 52 weeks at the GS-11. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. Individual Occupation Requirement (IOR): An IOR is a basic requirement that must be met in order to qualify for entry into this job series, and it is something that can't be waived. The education must be accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. To qualify for the GS 0671 occupational series, applicants MUST possess one of the following: You must have one year of specialized experience equivalent to at least the next lower grade GS-11 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Progressively responsible analytical or administrative, or clinical management or supervisory experience in the health care field. This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following: Missions, organizations, programs, and requirements of health care delivery systems; Regulations and standards of various regulatory and credentialing groups; and Government-wide, agency, and facility systems and requirements in various administrative areas such as budget, personnel, and procurement. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have Undergraduate and Graduate education Major study -- hospital administration, public health administration, or related fields such as business or public administration with course work in health care administration. OR, Special Provision for In-service Placement: Successful completion of an agency-sponsored on-the-job training program may be substituted for qualifying experience, provided it included a formal individualized training plan. Such a training program must have been conducted in an operating health care system and included: Assignments providing a knowledge of basic health system administration philosophies, practices, and procedures, and basic government administrative policies and requirements; Practical assignments providing an opportunity to apply health system administration skills and principles (as the individual progresses, work assignments must be characteristic of the grade level to which he or she is assigned); and Oversight by an experienced health system administrator with periodic evaluation of the individual's progress and appropriate adjustment of the training program. In addition to meeting the Individual Occupational Requirement as listed above, applicants must also meet the Specialized Experience, which is experience that is directly related to that of the position. Physical Requirements: The work is mainly sedentary and generally performed in an office setting. Typically, the employee sits at a desk or table, works on a computer, conducts group meetings, and makes group presentations. Some walking, standing and occasional moderate lifting are required.
