The Social Worker Program Coordinator is assigned to support the Veteran Directed Care (VDC) Programs and takes administrative responsibility for VDC clinical program development. This position serves as the VDC Program contact for both internal and external customers. The Social Worker Program Coordinator position location will be determined after selection is made and the incumbent may be allowed to utilize telework, as appropriate, to complete the requirements of the position.

Total-Rewards-of-an-Allied-Health-VA-Career-Brochure.pdf Social Work: vacareers.va.gov/wp-content/uploads/sites/5/Total-Rewards-of-a-Social-Worker-Career-Flyer.pdf VDC offers Veterans the opportunity to receive personalized in-home services and avoid placement into an institutional setting and remain at home through allowing the Veteran/Authorized Veteran Representative to develop a personalized Care Plan in coordination with the local Area Agency on Aging (AAA) (or other authorized VA provider) utilizing VA funds and receiving AAA assistance to budget for private hired care. The Social Worker Program Coordinator facilitates this through assessment for the appropriate case-mix level of care, providing referrals to the AAA and serves as point of contact for Veterans seeking enrollment into the program. The patients served live within the primary service area of NF/SGVHS (or have Patient Aligned Care Team (PACT) assigned in NF/SGVHS and live in a county that AAA provides service to). The Social Worker Program Coordinator may be required to travel to other locations of care within NF/SGVHS to carry out job duties/assignments. The Senior Social Worker is required to drive and/or travel using personal vehicle or government vehicle to other sites of care within NF/SGVHS, as well as, to community partners, as necessary to accomplish work assigned. Travel will be for long periods of time, up to three hours each way; travel is expected less than 25% of work week. The Social Worker Program Coordinator is assigned to the Geriatrics and Extended Care Section of Social Work Service (SWS) of the North Florida/South Georgia Veterans Health System (NF/SGVHS). Other duties include but are not limited to: responsible for coordinating the delivery of VDC Program services and care/case management of their assigned VDC enrolled Veterans performs independent clinical decision making, counseling, evidenced based psychotherapy, responding promptly to caregiving needs, financial issues, and end of life matters. provides Veterans and families with high quality case management, clinical treatment, advocacy, and coordinates linkage with appropriate VA and community service providers/agencies as needed by the client. utilize available data sources to proactively identify patients for assessment, treatment planning, case management and care coordination. independently conducts psychosocial and behavioral health/mental health assessments and can provide accurate diagnosis using most current version of the Diagnostic and Statistical Manual of Mental Disorders (DSM). responsible for recruitment and development of the Veteran Care Agreements conjointly with the Office of Care in the Community, recruitment, and marketing of the VDC Program development of annual budgets and care plans, ensuring budget and spending plans follow VDC program guidelines, and 180-day clinical oversight of review the status of each VDC enrolled Veteran. demonstrates the ability to teach and mentor staff and students in the GEC Service area of practice and to provide supervision for licensure or specialty certifications. Preferred experience: Experience in Geriatrics and Extended Care, with advanced level knowledge with advanced level knowledge and understanding of biopsychosocial, behavioral, and mental health needs of Veterans and families/caregivers. Care coordination and case management for Veterans, specifically in coordinating non-institutional services. Developing annual budgets and care plans, and review of budget and spending plans. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker Program Coordinator candidates must have the following: Experience/Education: Applicants applying at the GS-12 grade level must have 1 year of experience, equivalent to the GS-11 grade level. Program Coordinator social workers have experience that demonstrates possession of advanced practice skills and judgment. Social workers are experts in their specialized area of practice. They may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Training or additional certs must be submitted with application. AND Licensure/Certification: At this grade level, social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. They must have good manual dexterity. Must be able to express or exchange ideas by means of spoken and written word. Must be able to view and read information on computer screens. Work is typically performed in a clinical setting that exposes the incumbent to the communicable diseases and behavioral problems of the client population. The incumbent must be able to safely operate a government owned vehicle to be utilized in community-based work. Day-to-day work involves, sitting, walking, standing, carrying light objects and occasional driving for marketing events and meetings with local AAA and others (i.e., VA resource fairs, Ombudsman meetings, other Veteran organizations, and Center for Independent Living).
