The incumbent serves as a Social Worker Program Coordinator, locally titled Caregiver Support Program (CSP) Manager, which provides and oversees program services, education, coordination and resources to caregivers of Veterans with complex needs. The CSP includes the Program of Comprehensive Assistance for Family Caregivers (PCAFC) and the Program of General Caregiver Support Services (PGCSS) and reports primarily to the Gainesville VA Health Care System.

Duties

This position works closely with the Chief, Social Work Service on managing and overseeing all aspects of both the PCAFC and PGCSS day to day operations, to include, but not limited to fiscal management, data management, performance improvement, quality management, position resource management, and developing policies/procedures for program operation. The incumbent plans, organizes and coordinates administrative, operational, clinical and personnel activities for the CSP program. The incumbent is responsible for all aspects of program staffing, work assignments, budget and treatment services. The incumbent is responsible for managing and reporting on program specific fund control points, labor mapping, workload, and clinic utilization within the assigned program area. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. Provides oversight of facility CSP program fiscal matters of the functions supervised (which would include fund controls, contracts, and equipment expenditures), forecasts resource and equipment needs, and administers the allocated budget. The incumbent is required to travel between all NF/SGVHS locations and make home visits on a regular basis as required. The person selected for this position would need to report to the Gainesville VAMC or Lake City VAMC as requested and may also travel to the clinics as needed. The incumbent is required to drive and/or travel using personal vehicle or government vehicle to other sites of care within the NF/SGVHS, as well as, to community partners, as necessary to accomplish work assigned. Travel can be for long periods of time, up to three hours each way. Travel is expected at least 50% of work week. Duties include but are not limited to: Develops and facilitates CSP-program focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for VA facility staff and the community. Implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services (but who do not presently use VA for their health care needs). This includes outreach to current PGCSS and PCAFC participants to inform of available services. Outreach requirements include, but are not limited to, event planning for National Family Caregiver Month. Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Directly supervises the VA HCS Caregiver Support Coordinator(s) (CSC) and, when applicable, Program Support Assistant (PSA) and other interprofessional staff assigned to Caregiver Support Program Coordinates program implementation with multiple clinical services across all campus locations within the VA facility, including but not limited to: nursing; physical, occupational, and speech therapists; mental health and primary care providers. May provide clinical interventions to the Veteran and caregiver populations served by the programs. The incumbent is the subject matter expert (SME) to VA Facility leadership and staff for all CSP related issues and serves as a consultant to other VA professionals and administrators on program specific activities. The incumbent serves as a liaison to Veterans Integrated Service Network Program (VISN) CSP Program and the VA Central Office (VACO) Program. Applicants must meet the requirements by the closing date 05/31/2024 of this announcement. Basic Requirements: United States Citizenship: Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-13 Social Worker (Program Coordinator) candidates must have the following: Experience/Education: One (1) year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. The experience or education must be post-Masters of Social Work (MSW) degree. Experience and education satisfying this requirement must be active professional practice, which is paid/non-paid employment as a professional social worker directly related to this position. AND Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Advanced License must be submitted with application. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Preferred Experience: Ability to meet multiple ongoing deadlines in a timely manner. Ability to communicate both orally and in writing with staff at all levels of the organization, including service and facility leadership. Advanced skills with MS Office, to include Excel and Teams. Physical Requirements: The physical demands of the work are generally minor. The work is primarily sedentary in climate controlled clinical and administrative settings. The incumbent will be expected to successfully negotiate various functional requirements: lifting (15-44 pounds), carrying (15-44 pounds), walking (1-3 hours), reaching above their shoulder(s), the use of hands or fingers, sitting (4-6 hours), standing (1-2 hours), repeated bending, using stairs or elevators, functional vision, clear speech, hearing, and emotional stability. There may be occasional requirements to relocate boxed materials from one area to another, equipment and room set-up, and physical inspections of the facilities. This position requires travelling between all NFSG facilities and using a government vehicle so a valid state driver's license is required to operate government vehicles.
