The incumbent serves as in a developmental capacity at the lower grade level (GS-9). Serves as Facility PCMM Coordinator maintaining and continuously monitoring primary care panel sizes for each of the PACT Teams, adjusting panel sizes monthly using the calculated percentage of time each provider spends in primary care. Serves as Contracting Officer Representative (COR) for community-based outpatient clinics. Provides program oversight for the (CBOC's) in conjunction with Primary Care.
Duties
Duties and responsibilities include, but are not limited to:
Maintains and continuously monitors primary care panel sizes for each of the providers within primary care. He/she adjusts maximum panel sizes on a monthly basis using the calculated percentage of time each provider spends in primary care.
Routinely compiles and compares local data from VISTA on primary care statistics with national data in KLF to ensure accuracy of information. Any discrepancies are noted, analyzed and action plans for correction are developed and submitted to the primary Care Operations committee through the Chief of Primary Care.
Responsible for determining those patients who have not been seen by their primary care provider within the last two years and do not have a future appointment.
Responsible for reviewing the inactive lists to insure that scheduling mandates have been met. The information is also provided to the PCMM coordinator so that new assignments can be made for those providers who have not reached their maximum panel size.
An integral part of the primary care clinic cancellation process. He/she verifies for each primary care clinic cancellation request that there is an employee in the VISTA system, if appropriate. Reports are prepared by the incumbent on a monthly basis outlining number of clinic cancellations by provider and number of patients affected by the cancellation.
Coordinates and gathers data for the annual summaries of performance.
Generate quarterly clinical reminders data for each provider. Also, monthly clinical guidelines data by clinic. Generate queries on specific clinical guideline data as needed.
Monitor weekly data on unsigned progress notes. Data will be provided to the individual providers as a performance monitor with encouragement to reduce delinquent notes.
Monitor monthly productivity measures by provider. This data requires review of individual scheduled and a clear understanding of the clinic scheduling process. This data also requires manual extraction of data from clinic schedules.
Often called upon to prepare various type of information such as meeting minutes, correspondence, policies, procedures and medical center memoranda. Instrumental in assisting Chief, Primary Care with agenda, action items, etc for VISN conference calls, team meetings, provider meetings, etc.
Participates in all aspects of the Quality Improvement Program, develops and assists with the development of process monitors, indicators, standards and measurable criteria. In conjunction with the Business Manager of Primary Care, incumbent establishes methods and reporting mechanisms to track the status of monitors and indicators as they apply to primary care.
Assist with ensuring that courtesy and compassion is demonstrated to Veterans and their families, and accordingly is responsible for performance measures/customer services standards related to Primary Care patients.
Assist with developing effective relationships with the interdisciplinary team and individual staff, administration, other services to facilitate the delivery of patient focused care.
Exercises sound management practice and concepts to assure satisfactory program outcomes. Participate in analyzing VA directives and regulations and the impact on the various programs that encompass Primary Care Service.
Constantly attuned to the performance measures and strive consistently to improve performance in the areas of patient satisfaction, access for care, and timeliness on day of appointment, due to the number of Customer Service Standards related to the Primary Care.
Plans and develops training for employees, ensuring continuing in-service education and assessment of competency of employees and training needs in collaboration with professional clinical staff. Incumbent formulates and directs a formal training program to provide technically competent personnel to support the functions of the professional staff.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service.

The full performance level of this position is GS11; candidate may be selected at the GS9 grade level only.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: provides day to day management oversight of all administrative work and program activities for the department. Evaluates administrative/clinical programs and issues. Acts as a liaison between the healthcare facility and it's leaders.
