The Physical Therapist Clinical Specialist (Pelvic Rehab and Women's Health) serves as a consultant, clinician, and subject matter expert for the treatment of patients in outpatient Physical Therapy at the Bay Pines VA Healthcare Center in St. Petersburg, FL.

Duties

Major duties include but are not limited to: Performs physical therapy evaluations and treatments for female veterans ages ranging from adult to geriatric, having a variety of diagnoses. These cases may be complex in nature with chronic and acute disease entities including, but not limited to Pelvic Rehab, Women's Health Physical Therapy, chronic pelvic pain, pelvic oncology rehab, breast cancer post treatment rehab, peripartum and post-partum rehab, chronic and acute pain, orthopedic problems, joint replacement, central nervous system disorders, joint diseases, circulatory and pulmonary diseases, diabetes mellitus, and multi-system disease combinations. Serves as a physical therapy subject matter expert in the realm of women's health by providing in-services and programmatic support to the Physical Medicine and Rehabilitation Service (PMRS), Women's' Veteran Program, and to other services throughout the facility. Serves as a mentor for PT doctoral students, and other PM&R/physical therapy staff in clinical competencies pertaining to clinical specialty area. Evaluates patient's condition upon appropriate referral to physical therapy to determine treatment plan. Refers patients to Physical Medicine and Rehabilitation Service, Primary Care, or other specialty providers when determined necessary. Performs and interprets results of tests and measurements for use in preparing for, modifying, and/or evaluating patients for treatment, and adjusts treatment based on changes in patient's condition. Responsible for the Pelvic Rehab and Women's Health Physical Therapy clinical operation within the PMR department at the Bay Pines VAMC. Establishes and maintains appropriate Quality Assurance monitoring as related to outpatient physical therapy and the Pelvic Rehab/Women's Health Physical Therapy clinic, including, but not limited to patient outcomes, quality of care, and semi-annual documentation peer reviews. Documents patient performance toward achieving treatment goals, manages time appropriately, and assures a high level of productivity and work efficiency. Identifies continuing education needs based on performance of physical therapy staff regarding outpatient physical therapy and Pelvic Rehab/Women's Health Physical Therapy. Provides continuing education in the form of in-services and clinical training programs to other clinicians, including physical therapists, residents, students, and other medical center staff on the subject matter of Pelvic Rehab and Women's Health Physical Therapy. Serves as subject matter expert in Pelvic Rehab/Women's Health as a consultant to physical therapy and other medical center staff in evaluating and treating patients. Continues to learn advanced skills in this specialty area by attending and participating in continuing education courses and training, which further strengthens knowledge, skills, and abilities necessary to successfully perform and interpret evaluative procedures and tests, utilizing evidence-based practices and treatment for successful outcomes within this specialty area. Assists in the growth and development of the Pelvic Rehab and Womens' Veterans health and well- being programs at the Bay Pines VAHCS. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and experience: The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy. NOTE: Prior to 1995 the terminal degree for PT's was either a bachelors or master's degree. In 1995 CAPTE accredited all baccalaureate, master's, and Doctor of Physical Therapy terminal degree programs. Effective 2002, CAPTE changed its scope of accrediting activities of PT education programs to include only those programs that culminate in post-baccalaureate degrees(eliminating the baccalaureate degree). Effective 2015, professional physical therapy education programs have only offered the Doctor of Physical Therapy (DPT) degree to all new students. NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org. Foreign Graduates: Graduates of foreign physical therapy programs meet the requirements in subparagraph 3b if they have a full unrestricted and current license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. The Foreign Credentialing Commission on Physical Therapy (FCCPT) is a non-profit organization created to assist the United States (U.S.) Citizenship and Immigration Services (formerly INS) and U.S. state licensing authorities by evaluating the credentials of Foreign Educated Physical Therapists (FEPTs) who wish to immigrate and work in the U.S. Although the licensing of physical therapists in the U.S. is a right and responsibility of each jurisdiction, foreign educated individuals must undergo an educational credentials assessment for substantial equivalency to the first professional degree in the US. The Federation of State Boards of Physical Therapy (FSBPT) is committed to protect the public and to provide leadership within the field and practice of physical therapy. FSBPT's Foreign Educated Standards Committee has developed standards that represent "best practices" in evaluation services. The Credentials Evaluation Standards were developed with input from the physical therapy credentialing agencies that are licensed to use FSBPT's Coursework Tool. Licensure: Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetency or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the state for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed physical therapists on VA rolls as of November 30, 1999, provided the individual maintains a continuous appointment and is not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Loss of Credential: A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists. Grandfathering Provision: See education Physical Requirements: Pre-placement and periodic physical examinations are required for hybrid occupations to ensure workers are placed in positions where they can perform the essential functions of their job considering their physical, mental and emotional capacities, without endangering their health or the health of their co-workers. See VA Directive and Handbook 5019. English Language Proficiency: Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. See Chapter 2, section D, paragraph 5a, this part. Grade Determinations: GS- 13 Physical Therapist (Clinical Specialist): Experience: In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level and one of the following: 1. Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; or 2. Board Certification in the specialty area recognized by the ABPTS; or 3. Completion of a fellowship program; or 4. An additional advanced degree in a related field. In addition, the candidate must demonstrate the all following KSAs. Demonstrated Knowledge, Skills, and Abilities: 1. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy. 2. Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy (see paragraph 2i). 3. Ability to teach and mentor medical center staff in a specialized content area of physical therapy. 4. Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy. 5. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area (see Definitions, paragraph 2.i.). Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety. (see details in duties section) Preferred Experience: Preferred experience includes additional training and clinical experience in the evaluation, treatment and management of pelvic floor conditions, oncology rehab, and female orthopedic and geriatric conditions. ABPTS board certification in Women's Health preferred but not required. References: VA HANDBOOK 5005/99 PART II APPENDIX G12 The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is GS-13. Physical Requirements: The physical demands of the work require light lifting (under 15 pounds), light carrying (under 15 pounds), the use of fingers, reaching above shoulder, both hands required, walking (3 hrs.), prolong standing (6-8 hrs.), crawling (.25 hrs.), kneeling (.50 hrs.), repeated bending (2 hrs.), ability for rapid mental and muscular coordination simultaneously.

Grandfathering Provision. All licensed PTs employed in VHA on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: (1) Employees grandfathered into the GS-0633 occupational series as PTs may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation. (2) PTs who are appointed on a temporary basis prior to the effective date of this qualification standard may not have their temporary appointment extended or be reappointed on a temporary or permanent basis until they fully meet the basic requirements of this standard. (3) PTs initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, meet all the basic requirements of this qualification standard and must maintain the required credentials as a condition of employment in the occupation. (4) If PTs who are grandfathered under this provision leave the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
