This position is located in the medical department at Claremore Indian Hospital (CIH), Claremore, OK. This position is a coordinator and provider for pharmacy cardiology clinical, medication therapy management, and other assigned cardiology patient-centered pharmaceutical care services performed within the facility. The position reports to the Clinical Director.

Duties

Verify the patient's understanding of prescribed medications with patient counseling. Assure appropriateness of prescribed drug therapy through chart and patient screening. Provide drug information and health promotion/disease prevention information to patients and other healthcare professionals. Assist in the coordination and management of cardiology ambulatory care clinics: Provide guidance in scheduling, patient enrollment, space/staffing issues, coordination with dispensing processes, and other daily clinical activities as assigned. Develop and update evidence based, disease-state management protocols. Develop, implement, and maintain a pharmacist disease state management-credentialing program for each clinical program/service.

You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): ducation A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education(external link) (ACPE) or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. Licensure Applicants must be licensed to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience Applicant's qualifications background must demonstrate the knowledge, skills, abilities, and competencies necessary to perform the work of the position. Pharmacy work requires knowledge of the use, clinical effects, and composition of medications, including their chemical, biological, and physical properties. Qualifying professional pharmacy experience may involve, but is not limited to: Dispensing medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects; Evaluating medication use patterns and outcomes for patients in hospitals or managed care organizations; Performing administrative, consultative, or staff advisory work for a medical facility's pharmacy program; Planning, monitoring and evaluating medication programs or regimens; Establishing medication-handling procedures for the storage and preservation of medications; Researching medical literature and/or clinical medication information to provide accurate responses to inquiries; and/or Maintaining all medication records required by law. Basic Requirements for the GS-12 (or equivalent) Grade Level In addition to the requirements for the GS-11 (or equivalent) level, applicants must have a minimum of one year of professional pharmacy experience. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: GS-12: authority to adjust certain medications and order/interpret labs; performing drug usage evaluations and drug literature reviews to make clinical recommendations and formulary management for cardiac disease states. Experience in the development of clinic protocols, building templates and quick orders in EHR; experience interpreting and communicating/presenting complex disease state topics to a broad spectrum of interdisciplinary providers, nurses, leaders, and/or patients. Experience as a pharmacy preceptor providing education and training to pharmacy students and residents. Selective Placement Factor: Advanced Cardiac Life Support Certification (ACLS) and either Board Certified in Pharmacotherpy or completion of an Ambulatory Pharmacy Residency (PGY1). Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
