This position is located at the Edward Hines, Jr VA Hospital, Hines, Illinois 60056 (578) - a Level 1a Complexity hospital and is organizationally aligned under the Nursing Service. The primary purpose of the position is to serve as an Office of Community (OCC) Coordinator. The position functions as a non licensed member of the clinical care team who provide a robust interdisciplinary approach to care.

Duties

Medical Appointing and Clinical Care Coordination Support/Scheduling - The incumbent provides administrative and clinical care coordination support to clinicians and other administrative and professional staff for purposes of coordinating Non-VA medical care for veterans when inpatient and outpatient, and their VA provider has deemed such care necessary. - The Office of Community Care program requires administrative scheduling and appointment coordination for diagnostic testing, and consultative appointments for many clinical services. This position is responsible for generating authorizations for pre-authorized medical care. Works closely with the Community Care Coordination Nurse to ensure proper authorizations are issued and appropriate correspondence is sent to vendors. He/she acts as a liaison between the veteran and the vendors with regard to Community Care and VA clinical services and/or primary care services. Obtains requested medical records including reports, x rays, labs and any pertinent information required to complete the Office of Community Care (OCC) coordination consult in CPRS. Ensures all necessary components and documentation are distributed to appropriate services at the private institution where service will be provided. Collaborate with OCC nurse to prioritize requested procedure based on urgency (stat and routine), as dictated by Office of Community Care coordination policy. - The incumbent is responsible for the coordination of all administrative information between the professional staff and the patient. This includes verifying demographic information, data entry, administratively managing consultation requests and making and notifying the patient of his/her scheduled appointment with the outside provider(s). The incumbent is responsible for generating correspondence to the veterans and various providers and managing all received correspondence related to their care. Provides advisory and technical assistance to veterans and staff in answering questions related to OCC processes. Determines the nature of veterans' needs, questions or complaints. Incumbent utilizes HSRM, VistA, CPRS and other related systems to refer, manage and coordinate appointment management with the creation of authorizations as outlined by the Office of Community Care National Standard Operating Procedures. Works to identify and resolve appointment and/or procedure scheduling issues with veterans, vendors and clinical staff to ensure appropriate care is scheduled and authorized at community care facilities. Completes the Office of Community Care consult and scans or arranges for scanning all documents obtained into the VistA Imaging program assuring the corresponding report is linked to the appropriate consult, progress note title and patient. Employee works collaboratively with the hospital's managers, VA providers, discipline specific functional area professionals and community partners. - Makes administrative recommendations and program specific decisions that promote the efficient use of resources with the goal of improving timeliness and appropriateness of services provided. Develops continuous monitors dealing with both clinical and administrative quality assessment and improvement activities via program established customer service and quality management monitoring tools. Coordinates systematic reviews and gathers data for individual monitors. Reviews records for administrative criteria ensuring compliance with established standards. Advises clinical personnel and/or management of trends and recommends changes as appropriate. Incumbent will screen/receive phone calls in a courteous and timely manner, determine the nature of requests and provide the information desired using privacy rules and established processes. -Performs other related duties as assigned. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/04/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. Specialized Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-5) in the Federal Service that has given you the particular knowledge, skills and abilities required to successfully perform the duties of a Program Support Assistant: performing clerical/administrative duties in an office environment; answering and directing phone calls; preparing correspondence; maintaining files/records; and using a computer to perform a variety of support tasks. At this grade level, applicants should have good verbal and written communication skills and the ability to perform assignments independently with minimal oversight. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities.
