This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching, promotion, and education. This position provides care to Veterans seeking self-directed, lasting changes aligned with their values.
Position is located within the VA Medical Center in Spokane, Washington.
Duties

Duties include, but are not limited to, the following:
Provide interventions that promote health and wellness and enhance well-being, improve health-related outcomes, reduce the likelihood of inpatient admission, and improve quality of life
Display unconditional positive regard for their Veterans and a belief in their capacity for change and honor that each client is an expert on his or her life while ensuring that all interactions are respectful and non-judgemental
Work with Veterans to identify motivation for change, determine goals, develop/implement treatment plans, and assess progress in order to adjust plans
Provide information on the relationship between lifestyle and health outcomes
Collaborate with Veterans and the Veterans' primary care and specialty care treatment providers to develop goals designed to improve health outcomes, reduce risk from chronic illness, and lessen the likelihood of admission to inpatient care, long-term care, etc.
Contact eligible participants for enrollment and explain the health coaching process
Conduct interventions using one-on-one, couple, or group modalities in outpatient and inpatient settings
Conduct Whole Health Facilitated Groups (i.e., participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, exercise/fitness, etc.)
Provide follow-up care through telephone clinics or secure messaging
Utilize motivational interviewing and health coaching techniques and processes
Develop information for Veterans about the Whole Health Group and Individual Programs
Conduct outreach activities to inform of available programs
Provide educational opportunities to staff (i.e., lunch and learns, grand rounds, presentations at staff meetings, etc.)
Develop and distribute health education materials
Assist with data management analyses, special studies, and reports for use by management
Perform other related duties as assigned
Work Schedule: Monday through Friday, 8:00am to 4:30pm

Qualifications

Basic Requirements: You must meet one of the basic requirements below to be eligible for the Social Science Program Coordinator - Health & Well-Being Coach, GS-0101.
Degree: You must have successfully completed a bachelor's degree or higher degree, with a major study in behavioral or social science or related disciplines appropriate to the Social Science Program Coordinator - Health & Well-Being Coach position. Combination of education and experience: You must have a combination of education and experience that provided you with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. You should possess a core of educational credit in behavioral science, social science or a related discipline plus additional education and/or experience. The quality of the combination of education and experience must be sufficient to demonstrate that you possess the knowledge, skills and abilities required to perform work in the Social Science Program Coordinator - Health & Well-Being Coach position and is comparable to that normally acquired through the successful completion of a full 4-year course of study with a major in the appropriate field. Experience: You must have at least four (4) years of appropriate experience that demonstrated that you have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition to the English Language Proficiency and meeting one of the Basic Requirements above, to qualify for this position at the GS-9 level, you must meet one of the following:
Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-7) in the Federal Service that has given you the particular knowledge, skills and abilities required to successfully perform the duties of a Social Science Program Coordinator - Health & Well-Being Coach, and that is typically in or related to the work of the position to be filled. Specialized experience includes: Performing a variety of therapeutic and program related activities associated with assisting patients with identifying goals, developing behavior plans, and taking action steps to increase adherence to treatment for the purpose of enhancing wellness and assisting with the management of chronic disease; Conducting ongoing sessions to assess progress and assist clients in navigating obstacles or challenges; Conducting interventions for clients using one-on-one, couple, or group modalities; and Leading and conducting exercises for patients with chronic disease. Education: Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree that provided the knowledge, skills and abilities necessary to do the work of the Social Science Program Coordinator - Health & Well-Being Coach position. Such education may include courses in behavioral or social science, or related disciplines appropriate to the position. Combination: A combination of successfully completed graduate level education (beyond the first year) and specialized experience, as described above, which meet the total experience requirements for this grade level. This education must provide the knowledge, skills and abilities necessary to do the work of the Social Science Program Coordinator - Health & Well-Being Coach position. Such education may include courses in behavioral or social science, or related disciplines appropriate to the position. IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from geographic location of the facility to another. Some physical effort such as standing, walking, bending, or prolonged sitting may be required. The work places no special physical demands on the employee.
Work is performed in an office setting with occasional visits to areas within the service as well as throughout the Medical Center. Work area is adequately lighted, heated, and air conditioned. The normal risks and discomforts typical of an office and clerical work area are included. There are no unusual risks of safety hazards associated with this position.
