The Social Science Program Coordinator (Health and Well-Being Coach) position is supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing, and Whole Health clinical care elements of the Whole Health System. This position entails leading Gerofit exercise classes with the Geriatric Veteran population, including modification of exercises, patient evaluations and documentation.

Duties

***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** This position involves a multi-grade career ladder. The major duties listed below represent the full performance level of GS-9. At the GS-7 grade level, you will perform assignments of a more limited scope and with less independence. You will progressively acquire the background necessary to perform at the full performance level of GS-9. Promotion is at the discretion of the supervisor and is contingent upon satisfactory performance, availability of higher level work, and availability of funds. Major Duties Health Coach Duties: This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans. Works with Veterans on identifying the Veterans' motivation for change, determining goals for their work together, developing treatment plans to reach these goals, implementing and documenting those treatment plans in the Electronic Health Record, and assessing progress in order to adjust plans as necessary. This position entails leading group Gerofit exercise classes (per National VA Gerofit Guidelines) with the Geriatric Veteran population (age 65 years old and older), including modification of exercises, patient evaluations and documentation. Organizes the data collection and data management processes for exercise classes. Acts as Program Assistant for assigned Whole Health and Gerofit clinical project(s); supports, leads and participates in the activities of the team. The position also works with individuals and groups throughout the facility in a client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health and wellness. Coaches support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. Coaches assist clients to use their insight, personal strengths and resources, goal setting, action steps and accountability toward healthy lifestyle change. Health Coaches may work on inpatient units to assist with identifying goals, developing behavior plans for discharge, and assisting with post-discharge follow-up. Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings. Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals. Identify habits and behaviors that are harmful to quality of life and related to chronic illness. Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease. Reviews current health behaviors and develops customized treatment plans with Veterans. These plans include personalized goals, action steps and interventions. Schedule ongoing sessions to assess progress and assist in navigating obstacles or challenges. Guide Veterans to learn about available support tools in the VA and the community that assist in attainment of goals. Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness. Utilizes motivational interviewing and health coaching techniques and processes, learned through prerequisite health coach training, to assist members in developing a realistic plan to accomplish their goals. Coordinates with others on the Veteran's health care team to ensure optimal patient care. Performs other related duties as assigned. ***Preferred Experience: Experienced in Exercise Psychologist's or Whole Health Coach is preferred.*** Individual Occupational Requirements (Basic Requirements) Possess a degree: behavioral or social science; or related disciplines appropriate to the position. (TRANSCRIPTS REQUIRED) OR Possess a combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. (TRANSCRIPTS REQUIRED) OR Possess four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition to meeting the basic requirements above, you must also meet the Minimum Qualification Requirements below for this position. Minimum Qualification Requirements for GS-7 Superior Academic Achievement at the GS-7 level: (SAA) at the baccalaureate level in the coursework detailed above is fully qualifying. Applicants who wish to apply under this provision must submit proof from their educational institution that they meet one of the criteria listed below. To qualify for SAA, applicants must have completed all the requirements for a bachelor's degree in a curriculum described above for the position to be filled and meet one of the following: Rank in the upper one-third of your class in the college, university or major subdivision. A grade-point average of "B" (a GPA of 2.95 or higher out of a possible 4.0) for all completed undergraduate courses, or those completed in the last two years of undergraduate study. A grade point average of "B+" (a GPA of 3.45 or higher out of a possible 4.0) for all courses in your major field of study, or those courses in your major completed in the last two years of undergraduate study. Membership in a national honor society (other than freshman honor societies) recognized by the Association of College Honor Societies. OR Graduate Education: Possess at least one full year of graduate level study or possess a master's or higher degree in a field as stated above that provided the knowledge, skills, and abilities necessary to do the work. (TRANSCRIPTS REQUIRED) OR Specialized Experience: Possess at least one year of specialized experience equivalent to the GS-5 grade level in Federal service. Specialized experience is performing a variety of therapeutic and program related activities associated with assisting patients with identifying goals, developing behavior plans, and taking action steps to increase adherence to treatment for the purpose of enhancing wellness and assisting with management of chronic disease. OR Combining Education and Experience: Possess a combination of successfully completed graduate education and experience to meet total qualification requirements for the grade level. To be computed by first determining the total qualifying experience as a percentage of the experience required for the grade level; then determining the education as a percentage of the education required for the grade level; and then adding the two percentages. The total percentages must equal at least 100 percent to qualify an applicant for that grade level. The Gerofit aspect of work involves a wide range of physical demands from sedentary work in a standard office setting to moderate physical demands which include the ability to lead exercise and movement classes.
