The incumbent serves as a program coordinator for Dementia Care and Traumatic Brain Injury (TBI) providing outpatient case management and care coordination at the Salem Veterans Affairs (VA) Health Care System (HCS). This specialty treatment program is designed to provide preventative and proactive continuous patient-centered TBI and dementia care interventions that are evidence-based.

DUTIES:
Clinical Functions:
Veterans diagnosed with TBI and dementia and their caregivers providing case management, care coordination and supportive interventions as needed.
The incumbent will be called upon as needed to lead interdisciplinary team meetings to discuss challenging case issues, review case load status and provide feedback pertaining to complex cases. It will be the responsibility of the incumbent to track all TBI and Memory Disorder Clinic patients to ensure that regular monitoring is being provided to all participants of the program. The incumbent will enter patient/caregiver contacts in the electronic record using appropriate formats and templates.
The incumbent interviews the Veteran and their caregiver/family members/significant others to gather facts about the Veteran/dyad situation, presenting problem, and impact problems are having on Veteran's global level of functioning and health as part of the comprehensive bio-psychosocial assessment. The incumbent demonstrates knowledge of dementia related disease processes and related care issues ranging from early stages throughout later stages of dementia and end of life. Additionally, the incumbent demonstrates knowledge of Traumatic Brain Injury treatment and care.
Based on the psychosocial assessment, the incumbent uses professional judgment and advanced practice skills to make a psychosocial diagnosis. The incumbent assesses at-risk factors and high-risk cases and develops an individualized treatment plan for patients followed in the two programs. The incumbent makes independent professional decisions and collaborates with interdisciplinary team members as needed. The incumbent documents ongoing monitoring contacts, reassessments, evolving treatment plans, and alerts to inform other providers of changes in the patient's functioning and care needs.
The incumbent develops a psychosocial treatment care plan based on the obtained data from the psychosocial assessment. The incumbent develops treatment goals that address the unique needs of TBI and dementia patients who often have multiple medical and mental health issues. In formulating care coordination plan of care, the incumbent takes into consideration review of the medical record (VA and non-VA as appropriate), in person interviews, direct observation of patient/caregivers' presentation, consultation with other providers and the psychosocial assessment information. The incumbent assists the patient and caregiver(s) in developing a plan of care customized to specific individual care needs. The incumbent makes adjustments to the psychosocial treatment plan and interventions based on changing needs and response to treatment interventions.
Administrative Functions:
The incumbent is assigned to administrative responsibility for clinical program oversight and development. Duties include providing coordination of care and direct patient care services as well as managing the daily operation of the program. The incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN and national use. The incumbent manages and controls the use of data related to program and day to day functioning for program evaluation/ planning by maintaining statistical records for the program. The incumbent evaluates practice and identifies areas to improve services based on target outcome measures and proposed Medical Center benchmarks. The incumbent gathers relevant data, assesses the data and implements system changes based on findings to improve outcomes. This includes continuously developing and designing improvements for program efficiency and effectiveness and quality if social work services provided.
The incumbent has the ability to organize work, set priorities, meet multiple deadlines and evaluate assigned program areas. The incumbent uses sound judgment in the interpretation of trends and patterns, the application of practice guidelines and the recommendation of program enhancements. The incumbent will participate in activities that obtain feedback from patients and their family/caregivers to enrich services and interventions.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS 12:
Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.

Physical Requirements:
The incumbent must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands with reasonable accommodation, if necessary, without endangering the health and safety of the incumbent and others. Position requires walking throughout the Health Care Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans, their families, and interdisciplinary teams. Position may require coordinator to travel into the community to meet with Veterans and community providers. Incumbent must pass a recurrent driver's physical and maintain a current driver's license so that they are able to travel into the community and transport Veterans. This position also requires close visual acuity to perform an activity such as: viewing a computer terminal; extensive reading and typing; and repetitive motions of the wrists, hands, and/or fingers.
