This position serves as the Principal Facility Coordinator for PCMM (Primary Care Management Module). The Program Assistant will use PCMM functionality to maintain the database, perform research, prepare reports, troubleshoot problems. and test new patches. The Program Assistant will work closely with Medical Center's clinic support staff, clinical services chiefs, CBOC staff and contract providers, VISN and VACO staff.
Duties
The duties required to fulfill this position include, but are not limited to:
Utilizing a Graphical User Interface (GUI) to control the startup, setup and assignment functions and is also responsible for identifying inconsistencies and the ability to plan and communicate improvement activities across all disciplines.
Reviewing practitioner patient panels to determine the activity/inactivity of patients and follows appropriate procedures based on the research outcome.
Scheduling new patient evaluations to the Primary Care Providers.
Assuring that training, implementation, monitoring, and evaluation of all administrative procedures related to PCMM operations.
Serving as Consult Management Coordinator and working closely with Clinical Application Coordinators (CAC), Fee Basis Program, Clinic supervisors and leads to provide recommendations and guidance for consult requirements.
Monitoring clinic delays, access issues and research problems concerning consult scheduling and keeping management informed of issues impacting access.
Collecting, correlating and formatting data to allow for in-depth analysis of workload and patient wait times.
Monitoring and identifying opportunities using collected data sets such as trends in patient demographics, visit to Primary Care team, PCP and ER, inpatient Admissions and admissions to other facilities in the VISN.
Providing expertise and advice on a continuous basis to all hospital staff involved with PCMM by providing computer training on data processing procedures and protocols in use by the organization and in various software application of common data processing functions including standardized reports retrieval processes.
Performs other related duties as assigned.
Work Schedule: Monday-Friday 07:30am-4: 00pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 02/01/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. Proficiency Requirements for Office Automation positions - Typing Requirements: In addition to meeting experience or education requirements, applicants for Office Automation must certify that the candidate can type 40 words per minute.
You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-06 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
Performing work in support of general management and/or program analysis functions in which knowledge of the purpose, operation, methodology and techniques characteristic of specific management/program analysis functions
Communicate effectively with all team members, peers, patients and significant others, people from diverse cultural, educational, and organizational backgrounds within and outside the medical center
Obtain, organize, file and retrieve reports and numerical data for use in preparing status reports, briefings and presentations
Analyze workflow to determine potential problem areas and make recommendations to correct these areas and to improve coordination between the various providers, teams and clinics
Assign patients to Primary Care teams, removes patients from the database when discharged from Primary Care
Maintain accurate enrollment data, identify and correct discrepancies in the database system, and discharges from multiple or inappropriate specialty and consult clinics

Physical Requirements: The work is sedentary. There may be some walking, standing, bending, carrying of light items, such as papers, books, or small parts. May be required to stand for long period when conducting training sessions.
