The Connected Care Coordinator (CCC) is responsible for the planning and implementation of telehealth programs, identifying patient telehealth care needs at patient's site and negotiates with Service Chiefs and Specialty Care Providers to provide the service. His/her role involves working with a plethora of specialists and specialty services and must be adept at understanding the needs and requirements of the Specialty Care Provider to provide remote access to care for the Veteran.

Duties

As the Connected Care Coordinator, you serve as the telehealth consultant for all aspects of telehealth use, including facilitating training of the Telehealth Clinical Technicians and providers at the CBOCs and OPCs in the VA Healthcare System. Major duties include, but are not limited to: Training the personnel in each facility that will provide the hands on requirements for the interactive video encounters, in their role as educators for individual patients and participating physicians in this realm of consultation and care. Ensures that personnel engaging in telehealth activities meet the competency levels required by the VISN and the VHA Office of Connected Care. Provides leadership in evaluation and implementation of new clinical video telehealth and store & forward technology in coordination with the VISN Telehealth Coordinator. Provides consultation in the evaluation and implementation of new CVT technology in coordination with IRMS and the VISN Telehealth Coordinator. Serving as a clinical video technical telehealth consultant for all aspects of CVT and S&F telehealth use, including training of all stakeholders of telehealth (both consultation and treatment modalities) at the remote sites. Develops and manages clinical telehealth programming congruent with a comprehensive strategic plan and measurable outcome criteria. Works in close coordination with the VISN to assure that telehealth is used efficiently and appropriately to support a broad range of clinical functions, including specialty consultation, chronic condition management and educational endeavors. Chairs the Telehealth Committee and serves as a management specialist or subject matter expert on other local, VISN or National committees representing telehealth activities. Responsible for establishing clinic set-up in CVT and SFT at facilities, overseeing the scheduling into these clinics, facilitating the encounters between patient and physician, and ensuring the technical integrity of the encounter. Collaborates with the regional Office of Information & Technology and local Biomedical Engineering staff to ensure appropriate video quality specific to the type of encounter and assures availability and appropriate use of the peripheral devices necessary to conduct the encounter. Prepares and proposes an annual telehealth budget in coordination with Operations Staff or Chief of IRMS and VISN Telehealth Coordinator, as well as ongoing long and short-range acquisition strategic plan for VAHCS. Addresses essential aspects of legal and ethical issues in video-based care delivery in program planning and implementation. Consistently monitors utilization, fiscal impact, challenges, and outcomes of telehealth clinical applications. Seeks opportunities for appropriate program expansion, including external support and resources where indicated. Develops a specific data collection and analysis strategy for all aspects of telehealth program evaluation, including clinical, fiscal, and user and patient satisfaction criteria, and sees that evaluation is carried-out. Responsible for establishing clinics in CVT and S&F telehealth at each of the remote facilities, overseeing the scheduling into these clinics, facilitating the encounters between patient and physician, and ensuring the technical integrity of the encounter. Other duties, as assigned. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/24/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-12 position you must have served 52 weeks at the GS-11. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. General Adjustment SF-50's do not provide the information to reflect time-in-grade requirement has been met and therefore cannot be the only SF-50. Preferred Experience - Telehealth, Connected Care Experience. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-11 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: developing and managing clinical video telehealth and Store & Forward telehealth programming congruent with a population-based delivery of health care service which can meet the greatest clinical need, make the most efficient use of resources, and maximize user and patient satisfaction; broad administrative experience and understanding of healthcare systems and processes with familiarity with acute, long term, and ambulatory care delivery system; understanding of new approaches to healthcare delivery, and of national trends in clinical medicine toward decentralized care delivery. Physical Requirements: The work may involve some degree of physical exertion in setting-up and operating equipment. The incumbent may be subjected to stressful situations, as clinical video telehealth and store & forward telehealth systems can require immediate responses to unpredictable problems. Work hours may vary at times to accommodate clinical staff on irregular shifts. Work will be performed predominately in the systems facilities and its remote service areas. Considerable travel between various remote site facilities will be expected. Some travel to other VISN sites and attendance at necessary meetings will be required.
