The Management and Program Analyst provides support to the Primary Care Service at a VA Medical Center or Community Based Outpatient Clinics (CBOCs) within the Veterans Integrated Service Network (VISN) 15. The primary responsibility of the position is serving as the principal facility coordinator (PFC) of the Patient Centered Management Module (PCMM) and Newly Enrolled Appointment Requested (NEAR) Coordinator for all Patient Aligned Care Teams (PACT) within Primary Care Service.

MAJOR DUTIES AND RESPONSIBILITIES
GS-07 level, assignments are designed to prepare the incumbent to perform the duties and responsibilities in the target position of Management and Program Analyst, GS-0343-09. At this level, the employee plans and carries out routine work, making minor adaptations to procedures and accepted practices, and handling unexpected conditions arising in the normal course of the work but the level of knowledge required is less that what is required at the GS-09 level.
The PCMM is a complex, nationally implemented, stand-alone computer program that is designed to monitor and manage patients and staff in primary care, mental health, and home-based primary care services throughout the VA system.
The PCMM Principal Facility Coordinator (PFC) is the facility's designated staff member who manages and maintains the access, data entry, and accuracy of the PCMM.
Establishes and manages the PCMM PACT position set up, staff and FTE assignment, as well as patient assignments to clinics.
Meets regularly with facility Primary Care leadership and group practice managers to review provider panel management data to ensure patient assignments are made according to the current accessibility of the clinics and patient wait times, including access for face-to-face and telephone care.
Serves as the facility's technical expert for PCMM with the ability to troubleshoot and resolve user issues. In addition, oversees activities performed by PCMM Web application users performing in depth quality assurance.
Serves as the facility Newly Enrolled Appointment Requested (NEAR) Coordinator managing the NEAR List and process; provides training on NEAR process flows. Works closely with NEAR team members to appoint new enrollee appointment requests in accordance with VHA Directive 1230. Facilitates a warm transfer from enrollment to the NEAR schedulers for eligible Veteran walk-in enrollments who desire an appointment before leaving the facility.
Serves as the Primary Care Automated Data Processing Application Coordinator (ADPAC) and is responsible for completing and assisting with system access requests through ePas. Trains new users on their system access packages in VISTA and using the various function keys to ensure reports are ran and access to appointment management is available.
Serves as the Light Electronic Action Framework (LEAF) database administrator for the Primary Care Service
Ensures accuracy of PCMM through analysis and evaluation to ensure compliance with VHA PCMM and PACT policies, guidelines, and business rules. Develops, designs, and completes reports related to PCMM Web, PC and MH; utilizes the data to analyze and evaluate the performance, effectiveness, and efficiency of the PCMM Web. This responsibility is complicated by continually changing programmatic goals and strategic priorities. If data sources or reports do not exist, the incumbent collaborates with OI&T, Clinical Applications, and other Services to develop databases tailored to PCMM Web, PC, and MH.
Responsible for interpreting and analyzing applicable Primary Care PACT PCMM, Home Based Primary Care (HBPC) PCMM, Behavioral Health (BH) PCMM, Transition & Care Management (TCM)Team PCMM, Community Care/Primary Care & Specialty Care Service PCMM, State Veterans Homes PCMM and Traveling Veterans Coordinator (TVC) PCMM data to compile regular patient centered panel reports which includes information regarding the capacity of all primary care providers' panels and patient centered care panels.
Identifies and determines training priorities as they relate to PCMM Web, PACT, or any other areas of responsibility. Incumbent facilitates courses or presentations related to all aspects of PCMM Web and PACT. Gathers information to determine training needs, analyzes information obtained, and submits recommendations to supervisor.
Participates in PCMM training activities and in national and/or VISN PCMM conference calls and discussions as needed.
Performs other duties as assigned.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/24/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-5. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Gather, extract and audit data to produce reports utilizing a variety of computer systems such as VistA, Microsoft Office (Access, Excel and Advanced Excel formulas, PowerPoint, Word), Analyze and measure the effectiveness, efficiency, and productivity of medical center healthcare programs using knowledge of the mission, organization, and operations of VISN 15 facilities, Interpret, adapt, and apply guidelines to resolve gaps in specificity or conflicts in guidelines.

Physical Requirements: The work is sedentary in nature. The work sometimes requires walking, standing, bending, lifting, or carrying light items such as paper, books, and binders. Most work is performed in an office setting. Occasional visits to various patient care CBOCS and meetings may be required. No special precaution or protective clothing is needed.
