The incumbent serves as the GS-12 program coordinator for the Mental Health Residential Rehabilitation Treatment Program (MHRRTP) at the Clement J. Zablocki VAMC in Milwaukee, WI.

Duties

The Social Worker serves as coordinator of the admissions and screening processes for the MHRRTP Program, providing patient care, supervising unlicensed social workers and students, coordinating treatment programming with a multidisciplinary team, and developing and disseminating program policies and procedures. Responsible for the coordination of duties within the program, providing treatment to Veterans who are requesting residential mental health rehabilitation treatment services and referred from other departments within Mental Health, such as Veterans with a diagnosis of Depression, Anxiety, Substance Abuse Disorder, PTSD, Dual Diagnosis, etc. Employs a full range of social work treatment and assessment modalities with individuals, families, and groups with appropriate regard for the complexity of a given problem as assigned. Provides services at all points on the continuum of care from pre-admission to post discharge. Coordinates, facilitates, and participates in the screening and admissions process, maintaining appropriate patient flow, assisting case managers with discharge/transition planning efforts, including aftercare and follow-up referrals. Has extensive knowledge of aftercare options to include VA and community resources. Monitors the screening phone line for any self-referrals, attend the VISN Admissions Call and work closely with referral sources to coordinate care across facilities. Develops admission and screening procedures to improve timeliness and patient experience, tracks screening and admission data for program tracking and improvement purposes, maintains program datasets for reporting purposes to both local and national levels, tracks pending admissions and insure that weekly calls occur with Veteran awaiting admission, and arrange coverage when members of the screening team are unable to complete scheduled screenings or other duties. Collaborates with the MHRRTP teams, which consist of a multidisciplinary team of health care professionals and administrative staff that provide comprehensive accessible patient centered mental health care in partnership with the Veteran. Works collaboratively with MHRRTP managers on tracking and reporting on relevant national performance measures. Works collaboratively with the Suicide Prevention Team, maintaining responsibility for monitoring patients that may be at high risk for suicide. Works collaboratively with other Outpatient Mental Health stakeholders, including the BHIP Coordinator, the Recovery Coordinator, MST Coordinator, Post 9/11 M2VA , Evidence Based Treatment Coordinator, EAP Coordinator and other acute care staff. Works collaboratively with Acute Mental Health Staff to coordinate care when Veterans transition either to or from acute psychiatric/detoxification settings. Supervises discipline health professions trainees or practicum students in accord with training program policies and accrediting agency requirements. Performs other related duties as assigned. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. (COPY OF LICENSE REQUIRED) GS-12 Grade Determination: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
