The incumbent serves as the Primary Care Mental Health Integration (PCMHI) Program Coordinator and facilitates implementation of PCMHI initiative and strategies within the Clement J. Zablocki VAMC in Milwaukee, WI.

Duties

Responsible for the coordination of duties within the program, providing treatment to Veterans who are requesting Mental Health services through Primary Care, such as Veterans with a diagnosis of Depression, Anxiety, Substance Abuse Disorder, PTSD, Dual Diagnosis, etc. These services include provision of high quality evaluation, diagnosis, treatment planning, case management, brief clinical treatment, advocacy, and coordination of linkages to other appropriate VA service providers as needed by the Veteran. Provides a range of advanced interventions consistent with training and expertise, with special emphasis on brief and evidenced-based interventions found to be effective in primary care settings. Facilitates patient and family education appropriate to the primary care setting and the specific presenting mental or behavioral health issues. Provides consultation services for primary care providers to maximize the effectiveness of their interventions in reducing health-risk behaviors and enhancing health-promoting behaviors, with a significant emphasis on a prevention-oriented model of care. Supports primary care provider (PCP) decision-making, building on PCP interventions and improving PCP patient working relationships. Provides consultation and training to Primary Care Service Line staff in the identification and management of common mental and behavioral health disorders that present in a primary care setting. Lead team meetings, daily huddles and other meetings with program management as needed. Responsible for maintaining program statistics, performance improvement activities related to the PCMHI model. Works collaboratively with the Suicide Prevention Team, maintaining responsibility for monitoring PCMHI patients that may be at high risk for suicide. Works collaboratively with other Outpatient Mental Health stakeholders. Maintains a general knowledge of Veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. Serves as a member of various councils, committees and workgroups as designated, and participates in department/professional continuing education activities. Maintains responsibility for continuous PCMHI policy and procedure development and alteration, as necessary. Performs other related duties as assigned. Work Schedule: Monday-Friday 8:00 am - 4:30 pm Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. (COPY OF LICENSE REQUIRED) GS-12 Grade Determination: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. References: VA Handbook 5005/120 Part II Appendix G39 Social Worker Qualification Standard Physical Requirements: There may be some walking, standing, twisting, turning, sitting, pushing, bending, repetitive keyboarding, or carrying of light items.
