The Social Work Supervisor oversees a large complex program for all Seamless Transition activities and coordination of care and services for M2VA service members and Veterans treated at the Orlando VA Medical Center facilities, across all sites of care. The Coordinator/Supervisor also oversees the Veterans treated in the Traumatic Brain Injury (TBI) Program.

Duties

The selected Supervisory Social Worker facilitates efficient and appropriate delivery of services across episodes of care within the Medical Center and Community-Based Outpatient Clinics (CBOCs). Services include both medical and mental health services. The Program Coordinator/Supervisor plans, develops, implements, and evaluates the facility M2VA and TBI programs to ensure continual quality improvement and excellence in customer service. The work affects a wide range of agency activities and operations and directly affects the health and well-being of the patients served and relationships with community organizations and stakeholders. Some of the duties include, but are not limited to; Manage complex tasks that consist of monitoring Veteran treatment plans, educating and supporting staff, Veterans and caregivers, and providing oversite of all the components of the facility seamless transitions and TBI services. Manages the daily operation of both the M2VA and TBI Programs and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfaction. Develops a system to identify M2VA and TBI patients served by the facility and those in the catchment area not currently seen. Directly supervises the Social Work Case Managers, Transition Patient Advocate, DoD Liaisons, and other administrative staff (Program Support Assistant, etc) assigned to both the M2VA and TBI programs. Coordinates program implementation with multiple clinical services across all campus locations within the Orlando VA, to include nursing, Primary Care and Mental Health providers. Serves as the facility point of contact for all inquiries regarding seamless transition of Post 9/11 Veterans and as point of contact for M2VA Service Members and Veterans seeking health care from the facility. These duties include a wide variety of stakeholders (e.g., Veterans, families, state and community agencies, Veterans Service Organizations (VSOs), military support programs, etc.). Ensures M2VA and TBI Veterans receive comprehensive preventive mental and physical health evaluations. Promotes and coordinates evaluations for the M2VA and TBI Programs for the Medical Center and any outlying OPCs and CBOCs. Prepares program reports for VA Central Office, the VISN Point of Contact (POC), and Medical Center leadership. Develops procedures for referrals between the VA Medical Center, Military Treatment Facilities and/or community hospitals that promote appropriate and timely inter-facility transfers for both M2VA and TBI Veterans. Responsible for management of Military Treatment Facility referrals and the list of combat Veterans located in the facility's catchment area. Responsible for monitoring Veterans Health Administration (VHA) performance monitors and measures related to Post 9/11 Veterans to ensure they are met or exceeded. Develops policy and procedures to ensure compliance with The Joint Commission, Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards, as needed. Research community resources (local, state, and national) that are available to provide continuity of care and to enhance the quality of life of the veteran. Develops and implements a plan for outreach to extend services to M2VA and TBI Veterans who may benefit from care, but who do not presently use VA for their health care needs. Participates and coordinates outreach events to include Post Deployment Health Reassessment (PDHRA) and Yellow Ribbon Outreach Events as well as local community events. Establishes collaborative relationships with physicians, psychologists, nurses, and other disciplines to foster their involvement in the care of Post 9/11 and TBI Veterans. Other duties as assigned. Work Schedule: The primary tour of duty will be M-F 8am to 4:30pm. However, the selectee may also be required to work various shifts on a permanent or temporary basis, based on the needs of the service and patient care. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/03/2023. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Must be licensed or certified by a state to independently practice social work at the Master's Degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. (NOTE: For those who have a Florida LCSW license, you must be registered with the state as a "Qualified Supervisor" in order to provide supervision for licensure or specialty certification. All others must meet the requirements of their state of licensure to be able to provide supervision for licensure.) English Language: Must be proficient in spoken and written English Driver's License: A current, valid state-issued Driver's License is required for this position. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure or certification that are part of the basic requirements of the occupation. For employees who do not meet all of the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Such employees in an occupation that requires a licensure or certification, may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If an employee who was retained in an occupation listed in 38 U.S.C. § 7401 (3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. Grade Determinations: Supervisory Social Worker, GS-13 Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Preferred Experience: Those candidates with experience supervising multiple staff, ability to manage complex tasks, ability to monitor care coordination across programs, experience developing policy and procedures, and experience conducting outreach and presentations to community partners are highly preferred. References: VA Handbook 5005/120, Part II, Appendix G39 Physical Requirements: The position requires performance of work which involves light physical effort, such as that involved in lifting light files, bending, standing, reaching, operating office equipment such as telephone, copier, fax machine, scanner, etc. The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. In addition, must meet occupational health requirements for driving physical to include approval to transport Veterans in a government vehicle.
