This position is assigned to the Home-Based Primary Care (HBPC) program within the primary care at the VAMC, West Haven and the Newington, CT. The incumbent serves as Program Support Assistant for the HBPC under the supervision of the Program Director, HBPC. The program support assistant is responsible for organizing and maintaining the complex day to day administrative operations of the HBPC program.

Duties

The diverse responsibilities of the Program Support Assistant result in involvement daily with the Program Director, HBPC and other multidisciplinary staff members assigned to the program. This is a full-time position. Performs work involving the collection, compilation and/or tracking of date and statistical information in support of the HBPC's program operation. Assists in the planning, review, and reporting of data/statistical results of the program. Establishes and organizes computerized data sets and retrieves computerized data. Performs a basic analysis of the data and generates a variety of reports. Identifies initiates and organizes process for data collection for the program. Reviews documents and reports and ensures data entry is complete and accurate. Falls Tracking and Reporting: The incumbent works with the HBPC Falls Committee to ensure that all HBPC patient's falls are documented using the appropriate fall assessment form. All falls are tracked and presented to this committee quarterly. Data is compiled and submitted in a written quarterly report to the quality information section of the Office of Chief of Staff. Infection Tracking and Reporting: The incumbent works with all staff to ensure that all HBPC patient infections are reported. All infections are tracked as to who is responsible for the infection (HBPC, VA medical center, outside facility). In addition, all catheter and trach patients are monitored for infections separate from the rest of the HBPC census. As part of this tracking, all patients are monitored to ensure current vaccinations (only pneumococcal and influenza are reported) Data is then collected and submitted in report format to Infection Control/Medical Service on a quarterly basis. Timeliness Tracking: The incumbent monitors to ensure that all disciplines are seeing patients within the time frame set by the HBPC policy. This monitors the initial appointment(s) of social work, nursing, occupational therapy, nutrition, physical therapy as well as care plan completion and review by clinical pharmacist. Medication Reconciliation Monitoring: The incumbent works with nursing staff on each team to ensure that all HBPC patients discharged from the hospital receive medication reconciliation in a timely manner. Incumbent ensures that a spreadsheet/notebook is created and maintained that will have information on each veteran enrolled in the HBPC program. Maintains demographic information on all active patients within VISTA and CPRS. The incumbent works with HBPC Nurse Practitioners to ensure that all HBPC anticoagulation patients have current dosage letters and future appointments to comply with a primary care Quality Information monitor. Schedules all patients needing LPN services (medisets, injections, blood draws, BP checks etc.) Keeps accurate and up- to- date spreadsheet of these patients as well as schedules them via VISTA scheduling package. Assists in creating /updating policies for distribution to medical center. This includes researching material for policies and monitoring updates. Once reviewed by appropriate primary care staff, issue policy to medical center for concurrence. Monitor concurrences received and report appropriate primary care staff. Once completed, send completed policy with concurrence to Secretary to the Medical Center Director. Maintains files of all current hospital memorandums for reference by HBPC staff as needed. Monthly Reports: Includes monthly vesting report, average daily census, clinical visit tracking and total HBPC numbers. The monthly vesting report allows all staff to track the number of HBPC patients on their team that have reached their vesting goal each month. The average daily census is a report that is sent to the front office each week as a method for them to monitor our status and out progress toward our final goal. Consult monitoring: The incumbent will review each new consult for appropriateness for admission to HBPC program. This is done through a complete chart review; appointment history, patient location, and co pay status. If is determined that patient will be admitted to HBPC, incumbent identifies appropriate team members via view alert in Vista and/or CPRS. Then, all consults are monitored for admission or rejection to HBPC program by the Nurse Coordinator HBPC. Plans and executes educational in-services for HBPC staff covering a wide range of topics based upon medical center requirements and staff needs. Incumbent also serves as LMS administrator for HBPC staff. Work Schedule: Monday - Friday; 8:00am - 4:30pm Telework: Available in a Ad-hoc option Virtual: This is not a virtual position. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/13/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-6 level in federal service. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. You may qualify based on your experience as described below: Specialized Experience GS-07: You must have one (1) year of specialized experience equivalent to the next lower grade level GS-6 that has equipped you with the particular knowledge, skills and abilities to perform successfully the duties of this position, and that is typically in or related to the work of the position to be filled. (To be credible, specialized experience must have been equivalent to the next lower grade level in federal service) Examples of specialized experience include: Prior experience in applying analytical and evaluative techniques to the identification. consideration, and resolution of issues or problems of a procedural or factual nature; Knowledge of the CPRS/VISTA system, Event Capture, Patient Encounter Package, and other workload packages; sufficient to develop and maintain systems for workload capturing. Knowledge of the principles of management and organization including administrative practices and procedures common to organizations; Knowledge of a variety of VA policies and procedures, particularly related to the provision of patient care, is required. The incumbent must possess intimate knowledge of the State Home review criteria, the Community Nursing Home Oversight Handbook and OIG inspection criteria to ensure that the State Home, the community nursing homes and the medical center are in compliance with each. He/she must also be knowledgeable of Fee Payment Regulations to ensure compliance with VA policy and procedure. Ability to work independently with minimal supervision and solve problems independently; Ability to handle multiple tasks, including prioritizing competing demands; Skill in conducting interviews with supervisors and employees to obtain information about organizational missions, functions, and work procedures. Typing Requirement (For all OA POsitions) In addition to the Time in Grade and experience requirement, qualified candidates must also meet the requirement of 40 words per minute typing speed. Applicants may meet these requirements by passing the appropriate performance test, and presenting a certificate of proficiency from a school or other organization authorized to issue such a certificate, or, by self certifying their proficiency in detail on their resume. The VA Medical Center reserves the right to verify the proficiency skills of self-certified applicants by administering the appropriate performance test. Full performance level: GS-7 Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week.
