This position is for the Roseburg VA Healthcare System. The incumbent is the Primary Care Management Module (PCMM) Coordinator for the Primary Care Teams. This position is assigned to the Supervisory Decision Support Officer (SDSO), under the Office of the Director. This individual is responsible for maintaining the PCMM database for all of primary care throughout the healthcare system and ensuring the program reflects the current operational status.

Duties

Works in collaboration with the executive leadership team (ELT) team leaders, Clinical Managers, Providers, Nurses, Clerks, and the rest of the clinic staff, and provides oversight for PCMM functions in Mental Health, Home Based Primary Care, and all other specialty teams captured in PCMM. Provides support to and is involved in, optimal information management; system data gathering, analysis, validation, and presentation to recommend operational alternatives. Provides specific reports regarding timeliness and quality of care indicating panel sizes, clinic activity and workload patterns arid reports variances in clinic operations to Primary Care Management. Enters Primary Care and Mental Health team information into PCMM system. Assigns patients to Primary Care teams, removes patients from the database when discharged from Primary Care, maintains accurate assignment data, identifies and corrects discrepancies in the database system. Responsible for obtaining and maintaining accurate and timely information on provider-specific vesting compliance in Primary Care. This includes running reports and monitoring encounter form information pertaining to vesting. Obtains, reads, and keeps abreast of VA regulations and laws pertaining to requirements for elements included in data capture to ensure the inclusion and validity of Primary Care data transmissions. Works with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all providers and clerks involved with PCMM and provides expertise and advice to all staff involved with PCMM. Troubleshoots and coordinates processes with all staff involved with PCMM. Works along with administrative and clerical staff to implement systems and processes involving PCMM. Manages administrative functions associated with the PCMM program for the facility. Tracks and documents all administrative activities and requirements for the facility's Primary Care Management Module package. Oversees patient's administrative data inputted into PCMM. Obtains, organizes, files, and retrieves reports and numerical data for use in preparing status reports, briefings, and presentations. Identifies and recommends ways of eliminating, combining, simplifying, or improving procedures and processes. Performs other duties as assigned. Qualifications

To qualify for this position, applicants must meet time-in-grade requirements and specialized experience within 30 days of the closing date of this announcement, 10/16/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements within 30 days of the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Identifying and gathering information; communicating with internal and external clients; Using a variety of computer software; Knowledge of policies, guidelines, regulations, and precedents; Gathering, extracting, and analyzing data; identifying discrepancies and formulating solutions. NOTE: Experience must be fully documented on your resume and must include job title, duties, month & year starting and ending dates AND hours worked per week. You will be rated on the following Competencies for this position: ClericalCustomer Service (Clerical/Technical)Data Management Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: The work requires no special physical demands. It may involve some walking, standing, bending, or carrying of light items.
