The Intensive Community Mental Health Recovery/Mental Health Intensive Case Management (ICMHR/MHICM) Team associated with the VA Salt Lake City Health Care System is an interdisciplinary treatment team which provides intensive case management services to Veterans within the community diagnosed with severe and persistent mental illness. A substantial portion of the workload directed by this position may be carried out at one or more locations that are physically removed from the main unit.

Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as needed basis until positions are filled. *** Implements treatment modalities to achieve treatment goals using judgement and skills in utilizing supportive, problem solving, or crisis intervention techniques Performs specialized treatment of complex mental illness Incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including providing psychosocial and psychiatric interventions as an independent practitioner Establishes and maintains effective working relationships and communicates with Veterans, staff, and other parties as appropriate Functions as a case manager for Veterans in the ICMHR/MHICM program performing duties to facilitate independent community living by coordinating mental health and community care Collaborates with members of the ICMHR/MHICM team and other professional specialty clinics to provide comprehensive care to ICMHR/MHICM Veterans Utilizes knowledge of community resources, makes referrals to the community and other government agencies as appropriate, and coordinates services Identifies, develops, and facilitates accessibility and utilization of VA and community resources to meet the needs of ICMHR/MHICM Veterans Conducts psychosocial assessments and treatment interventions independently to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds Conducts individual and family clinical interventions to facilitate the identification of substance abuse, psychiatric illness, and social needs as treatment issues Assesses the ICMHR/MHICM Veterans' needs regarding their substance abuse, psychiatric and medical disorders, and the treatments required incorporating psychoeducation into the treatment Utilizes knowledge of medical and mental health diagnoses, disabilities, and treatment procedures to formulate a treatment plan Develops and participates in development of a treatment plan for each ICMHR/MHICM Veteran with measurable goals outlined based on relevant psychological, medical, and psychiatric information with the principles of treatment matching Develops and implements methods for measuring effectiveness of interventions utilizing outcome evaluations to improve treatment services in keeping with NEPEC standards and requirements Demonstrates the ability to design changes based on empirical findings Implements different treatment modalities in working with Veterans, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals Provides individual, group, and family interventions and advanced level case management interventions used in the treatment of Veterans with serious mental disorders Provides consultation, education, and advocacy Offers consultation to colleagues and students on the psychosocial treatment of Veterans treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills Demonstrates the ability to teach and mentor staff and students in the treatment of Veterans with complex mental illnesses Work Schedule: Monday-Friday 8AM to 4:30PM Telework: Ad-Hoc Virtual: This is not a virtual position Functional Statement #: 58381-A, 58382-A Relocation/Recruitment Incentives: Authorized Permanent Change of Station (PCS): Not Authorized Financial Disclosure Report: Not required

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred & tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken & written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education. Graduates of schools that are in candidacy status do not meet this requirement until the school is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Exception: VHA may waive the licensure requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure examinations. This exception only applies at the GS-9 grade level. For the GS-11 grade level & above, the candidate must be licensed. For appointments at the GS-9 grade level, VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within three years of their appointment as a social worker. Grandfathering Provision: All persons employed in VHA in this occupation on 09/10/2019 are considered to have met all qualification requirements for the title, series & grade held, including positive education & licensure or certification that are part of the basic requirements of the occupation. For employees who do not meet all of the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Employees in an occupation that requires a licensure or certification, may be reassigned, promoted up to & including the full performance level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to 09/10/2019 may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements must maintain the required credentials as a condition of employment in the occupation. If an employee who was retained in an occupation listed in 38 U.S.C. § 7401(3) under this provision leaves the occupation, the employee loses protected status & must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. Grade Determinations: GS-9: Experience, Education, & Licensure: None beyond the basic requirements Demonstrated Knowledge, Skills, & Abilities (KSA's): In addition to the experience above, the candidate must demonstrate all of the following KSAs: Ability to work with Veterans & family members from various socioeconomic, cultural, ethnic, educational, & other diversified backgrounds utilizing counseling skills Ability to assess the psychosocial functioning & needs of Veterans & their family members, & to formulate & implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, & assistance needed Ability to implement treatment modalities in working with individuals, families, & groups to achieve treatment goals. This requires judgment & skill in utilizing supportive, problem solving, or crisis intervention techniques Ability to establish & maintain effective working relationships & communicate with clients, staff, & representatives of community agencies Fundamental knowledge of medical & mental health diagnoses, disabilities, & treatment procedures. This includes acute, chronic, & traumatic illnesses/injuries; common medications & their effects/side effects; & medical terminology GS-11: Experience, Education, & Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) & licensure or certification in a state at the independent practice level. OR Education: In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, & Abilities (KSA's): In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of community resources, how to make appropriate referrals to community & other governmental agencies for services, & ability to coordinate services Skill in independently conducting psychosocial assessments & treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational & other diversified backgrounds Knowledge of medical & mental health diagnoses, disabilities & treatment procedures (i.e. acute, chronic & traumatic illnesses/injuries, common medications & their effects/side effects, & medical terminology) to formulate a treatment plan Skill in independently implementing different treatment modalities in working with individuals, families, & groups who are experiencing a variety of psychiatric, medical, & social problems to achieve treatment goals Ability to provide consultation services to new social workers, social work graduate students, & other staff about the psychosocial needs of patients & the impact of psychosocial problems on health care & compliance with treatment Preferred Experience: Experience with seriously mentally ill patients preferred. References: VA Handbook 5005/120, Part II, Appendix G39 The full performance level of this vacancy is GS11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS9 to GS11 Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, & carrying supplies. Must be able to perform primarily light & sedentary duties with occasionally moderate physical demands, exercise patience, & control emotions, with reasonable accommodation, if necessary, without endangering the health & safety of self or others. Position requires frequent travel into the community, usually around 80% of time, through use of a government vehicle or other individual arrangements, to meet with Veterans and representatives/stakeholders who help & support to Veterans in clinics, medical centers, Community Clinics, etc. Also, some travel is required to meet with VA community clinic staff, community agencies & partners who help & support to the ICMHR/MHICM Program & its Veterans. This position requires a valid state driver's license & local travel to provide community-based care to Veterans. Must meet eligibility criteria to drive a GSA vehicle. Also requires close visual acuity to perform activity such as: preparing & analyzing data; transcribing; viewing a computer; reading; & repetitive motions of the wrists, hands, and/or fingers.
