The Wilkes-Barre VA Medical Center is currently recruiting for one full-time Program Specialist to serve as the Primary Care Management Module (PCMM) Coordinator for the Patient Aligned Care Teams (PACT) across Primary Care Services. The Program Specialist reports directly to the Chief, Primary Care and works in collaboration with Chief Medical Officers, General Practice Managers, OEF-OIF Program Manager, Primary Care Providers, Nurses, Clerks and other clinical staff as appropriate.
Duties
The Program Specialist serves as the Primary Care Management Module (PCMM) Coordinator for Primary Care Teams within the main Medical Center as well as its Community Based Outpatient Clinics (CBOC). The Program Specialist is responsible for maintaining the integrity of the PCMM system database in all primary care settings throughout the medical center service areas. The Program Specialist works to help with training, implementation, monitoring, and evaluation of all administrative procedures related to PCMM operations as well as work closely with clerks in the Primary Care setting and OEF-OIF program facilitators, serving as a PCMM resource person.
Duties and responsibilities of the Program Specialist include but are not limited to:
Maintains the currency and accuracy of data/information in the PCMM system database;
Service as the initial point of contact for all relative inquiries to include but no limited to PCMM data capture and the availability and generation of related reports;
Maintains accurate enrollment data, dis-enrolling patients from multiple specialty and consult clinics, and discharges patients from database when dis-enrolled from Primary Care;
Tracks patient/provider assignments;
Assigns patient to primary care teams and providers dependent on current panels, diagnoses, special requests, etc.;
Generates a variety of reports on a daily, monthly, or quarterly basis;
Evaluates reports and presents findings related to trends, productivity levels, determinations for staff to work ratios;
Provides training, implementation, monitoring and evaluation of all administrative procedures related to PCMM operations.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/24/2023.
All Veterans Canteen Service (VCS) employees must have served in their Excepted Service position for at least 90 days in order to be eligible for a position within the Competitive Service.
Applicants who were previously selected under a Competitive Delegating Examining Unit (DEU) announcement must have served in their position for at least 90 days prior to being eligible for other positions within the Competitive Service.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.
You may qualify based on your experience and/or education as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Specialized experience includes: applying healthcare policies, procedures, and regulations to clinical functions, utilizing healthcare systems such as VistA, DHCP, CPRS, and appointment management software, applying oral and written communication skills; pulling reports; providing guidance and training to medical center staff; assisting internal and external customers; analyzing patient encounters and provider workload data related to Primary Care; making recommendations to management based on data; utilizing a wide variety of software for clinical data entry and retrieval; and applying clinical practices as they relate to PCMM, clinical creation, and maintenance. Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a master's or equivalent graduate degree or 2 full years of progressively higher-level graduate education leading to such a degree or LL.B. or J.D., if related. OR,
Combination: Applicants may also combine education and experience to qualify at this level. You must have a combination of specialized experience and education leading towards a Masters Degree.

Physical Requirements: Work is primarily sedentary but occasionally requires walking, standing, and carrying medium weight items.
