This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Duties

The HCHV Contract Residential/Grant and Per Diem Liaison in the Health Care for Homeless Veterans (HCHV) Program is an advanced practice social worker. The HCHV Contract Residential/Grant and Per Diem Liaison in the Health Care for Homeless Veterans (HCHV) Program incumbent is responsible for monitoring and coordinating services provided by community based programs funded under the HCHV Contract Residential Program and/or through VA's Grant and Per Diem Program. The HCHV Contract Liaison/GPD Liaison provides oversight services to ensure appropriate case management services are provided to program Veterans and assists with marketing and promotion of VA services at community events and participates in development of strategies to engage homeless Veterans. The incumbent is aware of community resources and provides information on housing, benefits and other local resources. Duties include but are not limited to: Monitors the clinical care and assesses the compliance of the programs receiving contract funds and/or per diem as described in the recipient's contract or original grant application. Intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction when necessary. Responsible for ensuring monthly billing is received from contractor/grantee, is reviewed for accuracy, and approved in a timely manner to ensure quick and accurate payment. Provides services and oversight of the funded programs as defined in the HCHV Contract Residential and GPD rules, regulations, and directives including verification of Veteran status and eligibility for participation in program, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, and collecting and submitting participant information as outlined by NEPEC evaluations procedures. Responsible for maintaining administrative files for each contract/grant. Administrative files include current contract/grant, fiscal documents (invoices, bed occupancy, payment history), and inspection documents. Coordinates and participates in quarterly and annual program site inspections in accordance with HCHV Contract and GPD Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices. When necessary, incumbent notes inspection deficiencies and establishes methods to monitor corrective actions. Incumbent recommends a response to the Medical Center Director as a result of inaction or insufficient actions related to the correction of deficiencies in accordance with the HHV Contract Residential and GPD Handbooks. Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless veterans as well as provides current information on services and benefits available in the VA for homeless program participants. Participates in HCHV Program performance improvement measures, including monitoring Veteran compliance with attendance at mandatory medical, mental health/substance abuse appointments. Compliance by HCHV Contract and GPD community providers with performance measures as stated in their contract/grant applications are monitored throughout the year and evaluated with the provider as part of the annual inspection report. Conducts and participates in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters, and "on the streets." Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the advanced practice level and must be able to provide supervision for licensure. English Language Proficiency Requirement: Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met. Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Physical Requirements: Must be able to sit at a desk to work at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The Clinical Assessor must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the social worker or others. Position requires walking throughout the clinic and/or Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. Position may require extensive travel into the community to meet with Veterans, caregivers, community agencies and/or partners who assist and support the Caregiver Support Program. Applicant must have a valid, active, unrestricted driver's license.
