The Veteran Directed Care (VDC) Program Coordinator is assigned to the Geriatrics & Extended Care Service. Is responsible for the professional & administrative management of the VDC for the VA Salt Lake City Health Care System catchment area. The VDC promotes patient-centered, non-institutional long-term care. The primary objective of the VDC Program is to provide enhanced options for veterans choosing to receive extended care services in the home & community instead of an institutional setting.

Plans, organizes, and directs administrative duties and oversight activities for all VDC community partners and programs under his/her responsibility. Coordinates daily operations, assists in planning, establishing and implementing policies and procedures, developing program goals and objectives, as well as monitoring, operating, evaluating and coordinating care with Veterans, families, VA staff and community partners. Acts as a liaison with community providers and identifying community resources. Provides continuous VA and Community staff education on program services, referral process, and the development, evaluation and communication standard, policies and procedures. Is responsible for all programmatic, administrative and placement related issues within the VDC program. This includes being a team lead for annual inspections/audits for the community providers, providing care in the VDC program, ensuring proper use of VHA VDC program guidelines, and assisting with congressional inquiries and problem areas of the program. Is a social worker who must be licensed at the advanced practice level and able to provide supervision of other dependent social workers and/or social work trainees as well as supervision for staff licensure. Provides services to patients residing in the home and community, who would, without those services, be in need of institutional care. Demonstrates independent clinical judgment and advanced decision making in the implementation of treatment plans. Has extensive skills in utilizing VA and community resources. Provides ongoing assessments of the Veteran's progress and treatment, as well as developing in-depth methods for determining the effectiveness of the program. Is responsible for weekly, monthly and/or quarterly meetings with National Geriatrics and Extended Care leadership related to VDC program. Is responsible for care coordination, case management services, service authorizations, program budgeting, quality improvement, program development, and consultation to others regarding the VDC Program. Works with VACO GEC Office Service and may serve as a mentor for other VDC sites throughout the nation. Is responsible for indirect supervision and coaching case managers in the community agencies to ensure professionalism and positive representation of VA. Provides ongoing training to new staff, community partners and caregivers. Is responsible for annual inspections/audits for each community partner agency. Establishes and maintains working relationships and agreements with local and state-wide Area Agencies on Aging (AAA). Responsible to organize and coordinate orientations on the program for representatives from community agencies as well as professionals working at VA medical center or community based clinics. Functions as the authorizing official for all service recommendations from the AAA. This requires implementation of a process for approving incoming expense reports and authorizing the release of funds to local AAA's who will in turn disseminate funds to veterans enrolled in the program. Streamlines the process for veterans who transition from traditional community home health services (homemaker, home health, skilled nursing, etc.) to the VDC Program. Assures that proper coding and billing practices are maintained, and negotiates basic and intermediate rates as well as types of services to be bundled for program participants with state AAA's. Works with AAA's to develop individualized treatment plans and assure that funding is optimally utilized for the purchase of a broad range of community services on behalf of beneficiaries and their families, while also developing and utilizing methods to improve quality of VDC program operations within the medical center. Seeks opportunities for program expansion by performing routine analysis of program services and groups that would benefit from VDC capabilities. Tabulates and analyzes data and statistics for the cohort of veterans served by the VDC Program. Participates in research activities as they pertain to enhancement of participant-directed services covered under the program, while also being available to perform other duties as assigned. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations Social Worker - Program Coordinator, GS-12: Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities (KSA's): In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. References: VA Handbook 5005/120, Part II, Appendix G39 Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying a health care bag and supplies. Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the social worker or others.
