This position functions both as the facility's LGBTQ+ Veteran Care Coordinator as well as a social worker in the substance use treatment program at the Salem VA Health Care System. providing case management, individual and group psychotherapy, crisis intervention and care coordination to this unit.

Duties

DUTIES: Some of the duties included with the LGBTQ+ Veteran's Care Coordinator are: Support the implementation of national and VISN-level policies related to LGBTQ+ Veteran health at the VA medical facility to ensure consistent and timely access to culturally competent care for LGBTQ+ Veterans. Ability to create local policies and procedures that are consistent with National and VISN policy to ensure that facility is in compliance with Directives. Communication with individual facility services (for example, prosthetics, endocrinology, social work, etc.) to provide tailored guidance and education as needed. This includes working with the clerical staff who may need training in asking about birth sex and self-identified gender identity in a respectful manner, and/or working with local Master Veteran Index coordinators (typically the Privacy Officer) for Veterans making changes to name and/or birth sex fields in the record system. Serve as a point-person, source of information, Veteran advocate, and problem-solver for LGBTQ+ Veteran-related health care issues at the VA medical facility. The LGBTQ+ VCC thus identifies the needs of local LGBTQ+ Veterans, assists the facility in developing needed care, advocates for a safe and welcoming environment throughout the facility, serves as a liaison with external LGBTQ+ community organizations, and develops relationships with internal facility stakeholders. Communication to the public via outreach measures, for example, sustainment of a dedicated Internet page for the facility's LGBTQ+ Veteran care resources and participation in community events and promoting a welcoming environment for LGBTQ+ Veterans (e.g., Website, advisory council, LGBTQ+ awareness posters, Pride events) that directly counteracts expectations of discrimination. Conduct LGBTQ+ specific monitoring and reporting in their VA HCS and related CBOCs. The LGBTQ+ VCC is the primary designee of the facility Director for monitoring and reporting on LGBTQ+ Veterans in the catchment area. Coordinate education and training for staff to reduce barriers to LGBTQ+ Veteran care to improve access to and quality of care at the facility. For the social worker in the substance use program, some of the duties include: Completes thorough psychosocial assessments and diagnostic interviews as part of the consult process when the Veteran is initially referred for mental health or substance abuse services. Other social work services include short- term, solution focused counseling and/or psychotherapy with individuals and families who may be in crisis, supportive counseling, and referral for resources and benefits. Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Completes thorough psychosocial assessments and diagnostic interviews to determine the psychosocial functioning and needs of Veterans and/or their families. Contributes to the development of the patient centered treatment plan and setting achievable treatment goals with the Veteran, family members/significant others and other outpatient mental health clinical staff. Maintains general knowledge of Veteran's benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies. Provides consultation and education to Veterans and their families and facilitates referrals based upon Veterans' needs and eligibility. Serves as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services. When appropriate and feasible, the Social Worker will educate and encourage the Veteran to advocate on his/her own behalf, thus fostering a sense of independence and empowerment. Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans and family members/significant others to the appropriate interdisciplinary team member for identified health education needs. Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, and discharge planning. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS12: Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.] Preferred Experience: References: VA HANDBOOK 5005/120 PART II APPENDIX G39 The full performance level of this vacancy is 12.
