The primary goal of HUD-VASH is to move Veterans and their families out of chronic homelessness.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Departments of Housing and Urban Development (HUD) and Veterans Affairs (VA) established the HUD-VA Supportive Housing (HUD-VASH) Program as a collaborative effort, supported through HUD Housing Choice rental assistance vouchers and VA's provision of intensive case management services. The primary goal of HUD-VASH is to move Veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve Veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated. The incumbent is responsible for program oversight, monitoring and administration, serving as the primary contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that Veteran care is coordinated with the VA Health Care System, community health or mental health providers, and agencies that serve homeless populations. The incumbent works closely with the Durham VA Health Care System's Homeless Programs, Residential and Community Care Programs to ensure that coordination between these entities is facilitated and Veterans direct care, referral, and follow up is contiguous and based on a continuum of services. The incumbent independently provides clinical psychosocial and case management services at an advanced practice level for eligible Veterans and their family members in the program. The incumbent has knowledge of theories and modalities used in the specialized treatment of complex physical or mental illness, and advanced skills in the use of treatment modalities including individual, group, and/or family treatment. Duties: Supervises employees to accomplish the work of the organization unit. Exercises supervisor authorities and responsibilities involving work assignment and review, as well as the administrative and personnel management functions relative to the staff supervised. Provides oversight with staff who review agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. Provides oversight to staff who verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation. Provides oversight to establishing a process for referral and admissions to the HUD-VASH Program, ensuring that homeless Veterans are considered as primary participants for program acceptance. Provides oversight to the collection and submission of HUD-VASH Program participant data as outlined by HOMES evaluation procedures. Provides oversight of HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. Provides oversight and coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Health Care System, VA Regional Offices and/or community agencies. Participates in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and health care system personnel on the bio-psychosocial aspects of homelessness. Takes responsibility improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Performs accountability functions: 1) document all contacts with patients/families in the electronic health record; 2) completes statistical, administrative, staffing studies and other reports in accordance with VA Health Care System and respective professional discipline service policies. Works with VA Health Care System multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD/VASH Program Veterans. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship. Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. ( d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Preferred Experience: MH/SUD/medical experience, coordinator/supervisory experience.
