This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
Duties
The Clinical Pharmacy Specialist serves with considerable independence in pharmacy activities under the general supervision of the Pharmacy Chief, or designee. This incumbent should keep supervisors apprised of trends or problems affecting any aspect of pharmacy activities, also, recognizing the need for changes in policy and procedures and makes viable recommendations. The pharmacist's work and service is reviewed on an overall basis by the supervisor for conformity to Pharmacy Service policy and requirements.
Administrative/Technical Support/Supervision
(1) Effectively communicates all appropriate information and significant events through proper channels, when appropriate.
(2) Correctly interprets and adheres to all hospital and Pharmacy Service policies and procedures.
(3) Continually develops and manages the Program and all assigned tasks.
(4) Accurately completes all assignments in a timely manner as defined by the Chief, Pharmacy Service or designee.
(5) Effectively manages and supervises technical support in the area of clinical and distributive responsibilities.
(6) Serves as a Subject Matter Expert (SME) on committees and workgroups as assigned (local NISN/national).
Clinical Pharmacy Care Activities
(1) Participate in multidisciplinary team rounds with the healthcare team(s).
(2) As part of the interdisciplinary team, appropriately render care to acute care patients, including, but not limited to, prescribing medications, to include initiation, continuation, discontinuation, and altering therapy, based upon established facility policies and protocols or under the Clinical Pharmacist Practitioner's Scope of Practice; physical assessment; monitoring medication regimens; ordering and analyzing necessary laboratory tests; adjusting medications; and assisting in the management of medical emergencies, adverse drug reactions, and acute and chronic disease states.
(3) Provide drug information, observations on patient response to therapy, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient outcomes taking into consideration choice of therapy, safety, efficacy, formulary, pharmacoeconomic, adverse drug reactions, allergies and disease states. Information and/or changes is communicated to the interdisciplinary team.
(4) Assist in preparing patients for discharge by evaluating the patients' medications, ordering necessary medications and providing education to the patient and/or caregiver.
(5) Evaluate patients admitted to the inpatient service for beta lactam allergies. Completes beta lactam assessments on these patients.
(6) Provide oral and written consultation with other health care professionals regarding drug therapy selection or management (including drug
induced problems) in an accurate, timely and professional manner. Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen.
(7) From a verbal or written patient presentation of signs, symptoms, laboratory tests and diagnoses, recognizes and lists important medical problems, disease states, symptoms or abnormal laboratory values that may necessitate altering the therapeutic regimen or which may be induced by drug therapy.
(8) Work to develop, upgrade, and disseminate acceptable criteria for use of formulary and non-formulary medications and other high-cost items directed at employing cost effectiveness without compromising the quality of patient care. Dissemination of information may occur in a setting requiring speaking in front of a large group of providers.
(9) All patients' interviews, assessments and interventions are documented and coded appropriately in a manner consistent with the current quality assurance policies.
(10) Design and/or participate in research studies (including drug utilization evaluations) with providers and/or other research collaborators.
(11) Respond and assist in emergency situations such as Code Met, Code Blue, etc. and maintains current BLS & ACLS certification.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
NOTE: This applies to current/past/upcoming VHA HPTs only. An HPT is an individual appointed to a temporary position in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or receiving VA Health Professional Scholarship Program. HPTs can be either VA-paid or Without Compensation (WOC).
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part.
Education.
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) [TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION]
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grade Determinations: GS-13 Clinical Pharmacy Specialist - Acute Care
Experience: In addition to the basic requirements, you must have 1 year of experience equivalent to the next lower grade level (GS-12)
Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
Knowledge, Skills and Abilities (KSAs) - In addition to meeting the experience requirements for this grade level, the candidate must fully demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Completion of PGY1 Pharmacy residency/ PGY2 Geriatric or Internal Medicine residency/ BCGP or BCPS Board Certification.
