This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one - five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
Duties
The Pain Management Clinical Pharmacy Practitioner (CPP) works collaboratively with primary care providers at the main facility in Danville, IL, and the Community Based Outpatient Clinics (CBOCs).
Serves as a mid-level provider with prescribing privileges as outlined in the individual's scope of practice.
Delivers evidence-based comprehensive medication management (CMM) through direct patient care and as part of the interdisciplinary team providing Pain Management.
Designs, implements, assesses monitoring and documents therapeutic plans utilizing the most effective, least toxic and most economical medication treatments.
Helps in achieving positive patient centric outcomes.
Maintains up-to-date knowledge of VA/DOD clinical practice guidelines, The Joint Commission (TJC) standards, Veterans Affairs (VA) performance measures, VA Pharmacy Benefits Management (PBM) initiatives and will integrate these standards into practice.
Participates as a subject matter expert on pain-related committees at the local facility level.
Assists with quality improvement projects related to pain.
Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the pain management team, as appropriate.
Identifies, documents drug errors, adverse drug reactions, allergies, and patient compliance issues per facility procedures.
Provides medication counseling/education to patients and caregivers.
Completes nonformulary request adjudications pertinent to pain management.
Provides clinical pharmacy consultations and drug information to professional staff on the pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics of pharmaceutical products.
Recognizes suicide risk factors, understands policies regarding suicide prevention and referral resources.
Establishes and reviews pharmacy policies concerning ambulatory care, acute care, and pain management.
Complies with all applicable Federal, VHA, and local regulations, policies and procedures pertaining to controlled substance accountability and drug accountability for pharmacy.
Precepts and evaluates pharmacy students, pharmacy residents, and other relevant trainees.
Promotes and monitors compliance for established drug therapy policies.
Performs other duties as assigned.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language: Must be proficient in spoken and written English
EDUCATION: Must be a Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
OR
Must be a Graduate of a foreign pharmacy degree programs that meet the educational requirement and able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). (DOCUMENTATION REQUIRED)
LICENSURE: Must possess a current, full, active, and unrestricted license to practice pharmacy in a State, Territory or Common wealth (i.e., Puerto Rico) of the United States, or in the District of Columbia.(MUST PROVIDE A COPY)
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-13 Level: You must have at least one (1) year of creditable experience (including relevant ASHP residency experience) equivalent to the GS-12 grade level VA or non VA which is directly related to the Pain Management Clinical Pharmacy Specialist position which includes, but are not limited to:
designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments;
helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas;
performing physical assessments;
ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
Pharmacists assigned to this position must only have demonstrated the following knowledge, skills and abilities (KSAs):
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
