The incumbent is a social worker and serves in a full-time capacity within the Mental Health Service (MHS) at VA Detroit Healthcare System which includes John D. Dingell VA Medical Center (JDDVAMC) and its affiliated community-based outpatient clinics (CBOCs), the Veterans Community Resource & Referral Center (VCRRC), and the VALOR Center.

Duties

The social worker is a member of the HUD-VASH Program and maintains approximately 70% of a full caseload with the remaining 30% of the time spent administratively as the team lead. Administratively, the incumbent is responsible for coordinating the staff's case management for a caseload of veterans in the HUD-VASH program. The Team Lead provides oversight, technical and clinical guidance to social work staff in the HUD-VASH program. The Team Lead facilitates multidisciplinary team meetings with at least weekly tracking of case conferences and at least weekly checks on charting compliance. The Team Lead assures that all Veterans in active case management assigned to their team have a team case conference recorded for input into diagnostics, treatment planning/reviews, and discharge planning to assure quality care. Clinically, the social worker is responsible for securing safe and affordable housing for Veterans provided through housing choice vouchers and supporting Veterans in maintaining their housing. The social worker provides psychosocial and intensive case management to Veterans enrolled in HUD-VASH, many of whom have a history of chronic homelessness. Veterans will be presenting with a broad spectrum of mental health, emotional, and behavioral concerns. Some work functions will take place at the John D. Dingell VA Medical Center, where the Homeless Programs Section is based, but much of the work (approximately 80%) is conducted in the community in order to best serve Veterans in the program. Provides case management for a caseload of Veterans in the HUD-VASH program (case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing) Provides direct linkage from community services to VHA homeless program services through regular and consistent outreach and communication with community providers Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies Performs site visits of HUD-VASH apartments within the Detroit metropolitan area (Macomb, Oakland, Sanilac, St. Clair, and Wayne counties) to ensure that Veterans reside in safe environments in compliance with local housing codes Performs site visits to Veterans within the Detroit metropolitan area (Macomb, Oakland, Sanilac, St. Clair, and Wayne counties) Verifies Veteran status, eligibility for VA medical care, and eligibility of program participation Conducts psychosocial assessments, identifies psychosocial problems and stressors, and diagnoses psychosocial and psychiatric conditions Reviews agreements between HUD-Housing Choice Vouchers, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements Collects and submits HUD-VASH Program participant data as outlined by NEPEC/HOMES evaluation procedures Participates in community and VA collaboration meetings, including, but not limited to By Name List Meetings assigned to each county covered within the catchment Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Team Lead Follows VHA, VISN, and local policies for the HUD-VASH Program. This includes following policies and documentation requirements as required for Commission on Accreditation of Rehabilitation Facilities Commission and Joint Commission accreditation Ensures treatment plans and clinical reminders are being completed appropriately and in a timely manner Reviews relevant data, assesses data, enters data in tracking systems including but not limited to HOMES and HMIS Participates in and leads regular reconciliation of HUD-VASH data between VA and PHAs to determine status of all awarded HUD-VASH vouchers Attends and participates in catchment county meetings as assigned Involvement in training program didactics for trainees of various disciplines and levels, as assigned Serves on Medical Center or Healthcare System, VISN, and/or National VA committees or task forces, as assigned by the Supervisor or the Associate Chief of Staff for Mental Health. Represents the Medical Center or Healthcare System at professional/program community group/events and committees. Qualifications

Basic Requirements: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations.
