The Mental Health Social Work Program Coordinator serves in a full-time capacity within the Mental Health Service (MHS) within the VA Detroit Healthcare System. The incumbent is part of the Mental Health Homeless Programs Section. Position may be filled as a Social Work Program Coordinator (HCHV Program Manager) OR Licensed Professional Mental Health Counsel (HCHV Program Manager)

Duties

The incumbent is responsible for the oversight of six distinct program areas which include the Community Resource and Referral Center (CRRC), Grant Per Diem (GPO), Contract Residential (CR), Veteran Justice Outreach (VJO), Prevention, and the Healthcare for Homeless Veteran (HCHV) walk-in and outreach clinics. Administrative Responsibilities Develops and implements regulations, policies, procedures, and protocols for the HCHV Outreach Programs including but not limited to CRRC service center and walk-in clinic, GPO, CR, Prevention, VJO, and HCHV walk-in and outreach clinics Supervises HCHV Outreach supervisors and frontline staff within the Grant Per Diem and Contract Residential Programs Has programmatic management responsibility for affiliated disciplines: psychiatry, psychology, nursing, occupational therapy, vocational rehabilitation, and peer support Provides executive oversight of the HCHV Outreach Program as defined in prevailing HPO directives and recommendations Represents the VA at community meetings, seeking or maintaining members on relevant boards, councils, and taskforces Plans, establishes, reviews, implements, and maintains policies, procedures, and protocols that satisfy accrediting guidelines (e.g. CARF, Joint Commission, VA-OIG, state regulation, etc.). Facilitates the development of HCHV Outreach program goals and objectives. Ensures the impartation and adherence to VACO, VISN, and local directives and/or guidelines that pertain to HCHV Outreach specifically and mental health generally. Is responsible for ensuring HCHV Outreach program compliance with local mental health/medical center policies and adherence to regional and national directives. Maintains the HCHV Outreach's consistent participation in Detroit VA Management System's mandates and expectations Collects and submits HCHV Outreach program participant data as outlined by VA Central Office and local Continuum of Care/Medical Center agreements Completes all HCHV Outreach suspense actions and reports Serves as an advocate on behalf of Veterans to ensure comprehensive service deliveryr Participates in local, regional, and national calls, conferences, training opportunities, seminars, and webinars pertaining to HCHV Outreach and/or HMLS Programs Maintains policies and documentation as required for all accrediting bodies including but not limited to CARF and Joint Commission Works with Homeless Programs Section Chief, MH Service Chief, and VA Detroit Executive Leadership Team to develop more effective and efficient ways of delivering healthcare services Coordinates interdisciplinary staffing and work assignments. Provides a full range of supervisory duties including performance evaluation, selection and training of staff, and recommendations for staff advancement or awards when warranted Manages the fund control points and budget line items assigned to the HCHV Outreach initiatives Is responsible for program needs such as material, equipment, and facilities needed to complete work. Administers information and analytical systems to evaluate and enhance the quality of work with HCHV Outreach Deals effectively with employees and union representatives on employee suggestions, complaints, grievances, and other matters involved in the day-to-day administration of the labor-management agreement Other Duties as assigned Clinical Responsibilities Provides clinical consultation and review of clinical interventions implemented by direct staff. Assesses high-risk factors, acuity, and the need for services Works with individuals with psychiatric and physical illness and disabilities, which may include acute infections, violent or aggressive behavior, and/or chronic psychiatric and medical problems Conducts individual and family clinical interventions to facilitate the identification of substance abuse, psychiatric illness, and social needs as treatment issues Conducts biopsychosocial assessments and assessment updates Education and Research Is involved in the didactic training of medical students, medical and psychiatric residents, nursing students, fellows, and other graduate or post-graduate trainees, as appropriate Other Significant Responsibilities The incumbent serves on Medical Center or Healthcare System, VISN, and/or National VA committees or task forces, as assigned by the Supervisor or the Associate Chief of Staff for Mental Health. The incumbent represents social work on decision-making committees at the Medical Center or Healthcare System, as assigned by the Supervisor or the Associate Chief of Staff for Mental Health. Qualifications

Basic Requirements: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 3 accreditation standards for a masters of social work. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). GRADE DETERMINATIONS. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-13 Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs. (a) Skill in coordinating and implementing one or more specialty program(s), across the facility. (b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. (c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. (e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. (f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. (g) Knowledge of organizational structure and impact on program operations. Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. At this grade level, social worker program coordinators oversee the administrative and clinical aspects of a major specialty treatment program, such as, but not limited to: Post Traumatic Stress Disorder, Substance Abuse Treatment Program, Home-Based Primary Care, Mental Health Intensive Case Management, Caregiver Support Program or Transition and Care Management Program, or a combination of programs located at one facility or multiple divisions of a facility. They are responsible for coordinating interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria 18 for the program. The program coordinator may or may not have supervisory responsibilities. They have full responsibility for all phases of service delivery for a major specialty program. The program coordinator is responsible for the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. They are administratively responsible for the programming and operations of the program assigned, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Program coordinators provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct program or service audits and complete designated clinical practice audits and reports, including productivity. Program coordinators oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement, and evaluating efficacy of improvement efforts. Physical Requirements: This position will involve driving a motor vehicle (with a valid driver's license), walking, climbing stairs, and carrying light objects while assisting Veterans in the Contract Residential agencies in the community. The ability to hear and communicate verbally with Veterans is essential. Work will take place indoors and outdoors. The work involves working closely with others. The incumbent must be able to lift/carry up to 50Ibs.
