The Ambulatory Programs Coordinator (APC) manages the administrative and clinical services of social workers and nurses assigned to four distinct program area - the Transition Care Management program, Social Workers assigned to Patient Aligned Care Teams (PACT), Oncology Social Worker and other Specialty Social Workers.
Duties
The APC is responsible for program coordination and supervision of the programs assigned. The incumbent manages responsibility for clinical and administrative operations at the John D. Dingell VAMC and two Community Based Outpatient Clinics located in Yale and Pontiac. Treatment services are provided in both an ambulatory setting and in home / community where the patient resides
The incumbent plans, develops, implements and oversees all components of outpatient social work practice.
Duties include, but are not limited to:
1. The incumbent manages information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, patient demographics, equipment utilization and patient satisfaction.
2. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
3. The incumbent executes the administrative and personnel management function relative to staff supervised. Incumbent establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system.
4. Promotes and coordinates evaluations for the the Medical Center and any outlying CBOCs.
5. Prepares program reports for VA Central Office, the VISN Point of Contact (POC), and Medical Center leadership.
6. Is responsible for monitoring Veterans Health Administration (VHA) performance monitors and measures related to PACT programming and TCM program to assure they are met or exceeded.
7. Establishes performance and outcome standards for the facility PACT/ Outpatient Speciality Social Worker's that promote quality and efficiency of services in coordination with the overall goals of the medical center and those of the VISN and VA Central office.
8. Develops a data driven continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness.
9. Develop a data monitoring system to measure the efficacy of the program.
10. Performs work related to the review, monitoring, analysis, planning and assessment of various programs. Responsible for key project activities related to the planning, development, and implementation of the project component. A significant portion of the workload directed by this position will be carried out at one or more locations.
11. Give advice, counsel, or instruction to employees on both work and administrative matters;
12. Hear and resolve complaints from employees, referring group grievances and more serious unresolved complaints to a higher level supervisor or manager.

Qualifications
Basic Requirements
Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part.)
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work
Licensure: Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part.
Grade Determinations: At the GS-13 grade, Supervisory Social Workers typically supervise professional and non-professional staff. They are responsible for the professional and administrative management of an area in a social work service, in a care/product line, or across multiple sites, (e.g., multi division facilities and CBOCs). Incumbent may also provide direct clinical services.
Experience: Individuals assigned as Supervisory Social Workers must have one year of specialized experience equivalent to the GS12 grade. level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience

Demonstrated Knowledge, Skills, and Abilities:
Ability to independently organize work, set priorities, and meet multiple deadlines.
Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
Ability to ensure provision of clinical social work services by supervised social workers.
Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion.
Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service.
Ability to provide staff training and development.

Preferred Experience: Management/ supervisory experience
Physical Requirements: The social work duties include sedentary and ambulation (walking) throughout the medical center and in the community according to the demands of daily tasks. Light carrying, less than 15 pounds-may be required as part of duties related to storing or moving files. Working around moving objects or vehicles-required as part of daily ambulating within the medical center around patients who use motorized scooters and wheelchairs. Working along-duties require working along at intervals during the day (i.e., documenting progress notes, completing phone call, etc.). Reaching above shoulder- may be required during daily task to access files located in overhead cabinets. Walking- required approximately 3-4 hours daily associated with walking from assigned office to outpatient clinics and other medical center departments as needed to complete duties of the position. Standing-required approximately 3-4 hours daily associated with walking from assigned office to outpatient clinics and other departments within the medical center as needed to complete duties of the position and filing patient records as needed. It is required that this individual be able to operate a motor vehicle as this position and it is expected that the incumbent visit patients off-site as part of their duties. Work will take place in- and outdoors thus exposing them to the natural elements of the weather.
