This developmental position is located in the Primary Care Service at the Jesse Brown VA Medical Center in Chicago, IL. The Connected Health Coordinator serves as a facility champion for Connected health, leads facility teams to support existing and future Veteran communication initiatives, coordinates local implementation, training, communications, outreach, and support efforts, and evaluates and reports on Connected Health actions and activities.

Duties

- The incumbent leads the facility's efforts and directs activities to support current and future Connected Health modalities (i.e., mobile health applications [apps], patient generated health data [PGHD], MHV, SM, and VHRC) with healthcare providers and patients and serves as liaison and administrator to national and facility teams. - Develops mechanisms to evaluate and monitor the quality, safety, efficiency, utilization, productivity, and effectiveness of Connected Health. - Works collaboratively with the VISN Telehealth Technology Program Manager (TTPM) and supports plans, organizes, directs, evaluates, and reports Connected Health-related actions/activities and provides authoritative guidance and technical coordination in establishing network Connected Health-related standards, goals, protocols, and monitor/report outcomes. - Responsible for establishing and managing communications plans that identify target audiences including appropriate messages, activities, and resources. - Analyzes functional clinical and business process changes or impact to clinical guidelines in terms of policies, procedures, and user behaviors. - Engages clinical, business, and operational staff through analysis and recommendations focused on the fact that programs are not just new business practice techniques and strategies. - Provides leadership in evaluation, implementation, and management of all new and existing Connected Health technologies. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/22/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. For a GS-11 position you must have served 52 weeks at the GS-09. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. Basic - Individual Occupational Requirement: A. Education: I possess a bachelor's or graduate level degree in a major field of study such as hospital administration, public health administration, or related fields such as business or public administration with course work in health care administration. OR B. Specialize Experience For positions above GS-05: I possess Progressively responsible analytical or administrative, or clinical management or supervisory experience in the health care field. This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following: 1) Missions, organizations, programs, and requirements of health care delivery systems;2)Regulations and standards of various regulatory and credentialing groups; and 3) Government-wide, agency, and facility systems and requirements in various administrative areas such as budget, personnel, and procurement. OR C. Special Provision for Inservice Placement: Successful completion of an agency-sponsored on-the-job training program may be substituted for qualifying experience, provided it included a formal individualized training plan. Such a training program must have been conducted in an operating health care system and included:1)Assignments providing a knowledge of basic health system administration philosophies, practices, and procedures, and basic government administrative policies and requirements;2)Practical assignments providing an opportunity to apply health system administration skills and principles (as the individual progresses, work assignments must be characteristic of the grade level to which he or she is assigned); and 3) Oversight by an experienced health system administrator with periodic evaluation of the individual's progress and appropriate adjustment of the training program. You may qualify based on your experience and/or education as described below: GS-09 Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: progressively responsible analytical or administrative, or clinical management or supervisory experience in the health care field. This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following: missions, organizations, programs, and requirements of health care delivery systems. Specialized experience also includes: knowledge of overall functions of the Medical Center, including knowledge of its responsibilities and Veteran Population served; knowledge of polices which affect the Facility and have an impact on the administrative functions; knowledge and skill in applying analytical and evaluative methods and techniques to issues concerning the efficiency and effectiveness of program operations carried out. GS-11 Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-09 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: progressively responsible analytical or administrative, or clinical management or supervisory experience in the health care field. This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following: missions, organizations, programs, and requirements of health care delivery systems; regulations and standards of various regulatory and credentialing groups; and Government-wide, agency, and facility systems and requirements in various administrative areas such as budget, personnel, and procurement. Specialized experience also includes: knowledge of overall functions of the Medical Center, including knowledge of its responsibilities and Veteran Population served; knowledge of polices which affect the Facility and have an impact on the administrative functions; knowledge of computer programs including word processing, communications, graphics, and spreadsheet development; ability to collect and analyze data, create data bases, prepare statistical summaries, draw conclusions, and recommend appropriate actions that are related to the mission. To qualify based on education for this grade level you must have successfully completed a master's or equivalent graduate degree or 2 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related. Graduate education must be in a Major Study- hospital administration, public health administration, or related fields such as business or public administration with course work in health care administration. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work. (TRANSCRIPTS REQUIRED). OR

GS-09 Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond a Masters degree.

GS-11 Education: Applicants may substitute education for the required experience.
